• Etymologie: Moederkruid = kruid voor moeders, voor bevalling, Parthenium = van het parthenon of van parthenos, jong meisje / Feverfew = featherfew (gevederd blad) of van fever (koorts) / Tanacetum = van grieks athanasia (onsterfelijk)

• Genus: vroeger Matricaria parthenoides L., later Chrysanthemum parthenium, nu Tanacetum, zoals T. vulgare (Boerewormkruid)  en T. balsamita (Balsemwormkruid, Vrouwenmunt)

• Variëteiten: T. parthenium ’Crispum’ (sterker gekruld blad)

• T. parthenium ’Aureum’ (gelig blad)

• T. parthenium variêteit met alleen witte lintbloemen

• Beschrijving: Vaste plant (Hemikrypt) met sterke geur en veerlobbige geelgroene bladeren tot 8 cm lang, bloeit in juni en juli met kleine (1cm) margrietachtige bloemen. Gekweekt en verwilderd op bouwland, braakland, nabij dorpen. Is makkelijk te vermeerderen door zaaien.

Materia medica, gebruikte delen

• Tanaceti parthenii herba / folium, het hele kruid of het (jonge) blad van vooral de verse plant Tanacetum parthenium Schultz - Bip. en mogelijk ook variëteiten.

• Oogst: van het blad kan van mei tot november, het is mogelijk 3 maal per jaar te oogsten

Samenstelling

• ** Sesquiterpeenlactonen vooral parthenolide (Soucek 1960) maar ook germacreen, beta-farnaseen, costunolide ea. (overzicht Bohlmann & Zdero 1982), deze stoffen hebben een bitterwerking en zijn lipofiel (lossen vooral op in alcohol en olie).

• * Flavanoîden, vooral luteoline en apigenine (Greger 1969)

• * weinig etherische olie met kamfer en chrysanthenylacetaat (Depooter e.a. 1990)

Farmacologie

• ** Ontstekingswerend (anti-inflammatoir) en pijnstillend door remming van ontstekingsmediatoren zoals prostaglandinen (PGE2) en thromboxanen, min of meer te vergelijken met werking van aspirine, remt ook allergie uitlokkende stoffen zoals histamine en serotonine, afscheiding van serotonine uit humane bloedplaatjes wordt geremd (Heptinstall e.a. 1985), kan ook een rol spelen bij migraine (Hanington), uitscheiding van enzymen uit PMN’s (soort witte bloedcellen), die schade veroorzaken in de gewrichten, wordt verminderd (anti-reumatisch), tromboxane A2, dat de arteriele vaatwanden samentrekt, wordt door parthenolide geremd (krampwerend op de bloedvaten)

• * remmende werking op aggregatie van de bloedplaatjes (Heptinstall 1985)

Indicatie / Gebruik van Tanacethum

Receptuur en bereidingswijzen

• Vers kruid voor de bloei opeten, 60 mg of 2 blaadjes daags, gedurende 2 maand. Vers kruid kan in 6% van de gevallen irritatie van het mondslijmvlies veroorzaken. Het jonge blad bevat waarschijnlijk het hoogste gehalte aan parthenolide (beste werking)

• Thee? weinig gebruikt, werking niet duidelijk

• Gedroogd kruid, eventueel in capsule (gaat irritatie tegen), 25 mg 2 maal daags.

• Extracten met gestandariseerd parthenolide-gehalte eventueel te combineren met teunisbloemolie (stimuleert PGE1, Moederkruid remt PGE2)

Geschiedenis en wetenschappelijk onderzoek

• Dioscorides (1ste E.) vele toepassingen maar ook voor ‘hete inflammatie en hete zwellingen’ wat misschien al verwijst naar artritis.

• Dodonaeus: beschrijft Moederkruid als middel om het afdrijven van nageboorte te vergemakkelijken. Cruydtboeck 1554.

• Gerard John (1633) : ‘ very good for them that are giddie in the head (=hoofdpijn, migraine?)

• John Hill: ‘in the worst headache this herb exceeds whatever else is known’ (Family Herbal 1772)

• Culpeper (1649): adviseert Feverfew voor vrouwen als ‘a general strengthener of their wombs’ en zegt dat het ‘very effectual is for all pains in the head’.

• Ravelingius: ‘ Voor hoofpijn door koude veroorzaakt’ neem het groen gestootte kruid en legt het op de kruin van het hoofd.

Historique de la grande camomille

Contrairement à ce que pourrait laisser croire son nom anglais, la grande camomille n'est pas une plante qui prévient ou réduit la fièvre (fébrifuge). Le nom Feverfew serait plutôt une déformation de featherfoil, une allusion à la forme de la feuille qui rappelle celle d'une plume. Traditionnellement, on l'utilisait surtout contre les maux de tête, l'aménorrhée (l'absence de menstruations) et les douleurs rhumatismales. Les autres usages traditionnels répertoriés comprennent l’arthrite, l’asthme, les douleurs menstruelles, les maux d’estomac, les maux de dents et les piqûres d’insectes.

Son usage est tombé en désuétude, mais l'histoire veut que l'intérêt scientifique pour la grande camomille ait resurgi vers la fin des années 1970. La femme d’un haut responsable des services de santé de la commission nationale anglaise du charbon souffrait de fortes migraines récurrentes. Après que la dame eut essayé les feuilles de grande camomille, employées avec succès par un mineur, ses migraines cessèrent presque complètement. Le Dr Stewart Johnson de la London Migraine Clinic amorça par la suite une série d'essais cliniques qui contribuèrent à établir la réputation de la plante.

Recherches sur la grande camomille

Efficacité possible Prévention de la migraine. Une synthèse systématique publiée en 2004 a porté sur 5 essais (343 sujets en tout)1. Les auteurs estiment que, dans son ensemble, la preuve n’est pas convaincante. En effet, les essais concluants sont de faible qualité méthodologique selon eux et, au cours des deux essais présentant la meilleure méthodologie, la grande camomille n’a pas fait mieux qu’un placebo pour prévenir la migraine. Selon une étude globalement non concluante, il est tout de même possible que la grande camomille soit efficace chez les personnes dont les migraines sont fréquentes2. Certains cliniciens considèrent que la grande camomille mérite d’être essayée, notamment lorsque les médicaments classiques ne peuvent être utilisés3.

Une étude de bonne qualité publiée en 2005 a donné des résultats positifs. Les 170 participants ont pris durant 16 semaines soit un placebo, soit un extrait de grande camomille (MIG-99®), à raison de 6,25 mg, 3 fois par jour. Dans le groupe traité, les crises de migraine ont été moins nombreuses que dans le groupe placebo4.

D’autres produits contenant de la grande camomille ont été utilisés avec succès au cours de deux essais : l’un contenait, en plus de la grande camomille, du gingembre5 et l’autre du saule blanc6. Mais le fait que ces essais ne comportaient pas de groupe placebo, ainsi que le faible nombre de participants limite la portée de leurs résultats. Une étude avec placebo portant sur un autre produit combiné (grande camomille, riboflavine et magnésium) n’a pas été concluante7.

Théoriquement c’est la présence du parthénolide qui explique l'action préventive des feuilles de grande camomille. Cependant, les experts pensent de plus en plus que cette substance est loin d'être la seule en cause et que sa présence n'est pas une garantie d'efficacité. En fait, tout ce qui a été clairement démontré, c'est l'efficacité des feuilles séchées ou lyophilisées de la variété anglaise de grande camomille (Tanacetum parthenium (L.) Schultz-Bip.)8.

Usage reconnu L'ESCOP et l’Organisation mondiale de la Santé reconnaissent l'efficacité des feuilles de grande camomille pour la prévention de la migraine.

Divers. Bien que la grande camomille ait été traditionnellement utilisée pour les douleurs rhumatismales, la seule étude clinique ayant testé son efficacité pour soulager l’arthrite rhumatoïde n’a pas été concluante9.

Attention

La camomille allemande (Matricaria recutita, M. chamomilla), qui est une plante totalement différente de la grande camomille, n'a pas du tout les mêmes propriétés que cette dernière et n'est, par conséquent, pas utile pour prévenir la migraine. Elle ne peut en aucun cas se substituer à la grande camomille.

Contre-indications

Allergies. Les personnes allergiques aux plantes de la famille des composées ou astéracées (la marguerite, l'échinacée, le pissenlit, etc.) devraient éviter la grande camomille.

Grossesse. La plante étant traditionnellement réputée pour son efficacité à déclencher les menstruations, on conseille aux femmes enceintes de l'éviter.

Effets indésirables

Très rarement, de légers troubles digestifs.

Chez certaines personnes, le fait de mâcher les feuilles peut causer de petits ulcères buccaux, généralement après plusieurs mois d'usage continu. En général, on peut contourner ce problème en prenant des capsules ou des comprimés au lieu de mastiquer les feuilles.

• Selon des données in vitro, la grande camomille a des effets antiplaquettaires. Théoriquement, ceux-ci pourraient s'ajouter à ceux d'autres plantes ou suppléments à l'action anticoagulante ou antiplaquettaire.

Tanacetum parthenium

Chrysanthenium parthenium (L.) Bernh. (= Matricaria parthenium L. = Pyrethrum parthenium (L.) Sm. = Tanacetum parthenium (L.) Schultz-Bip.)

Pour les botanistes, la grande Camomille porte plusieurs synonymes : Tanacetum parthenium, Chrysanthemum parthenium, Leucanthemum parthenium ou Pyrethrum parthenium.

Originaire d'Asie Mineure et des Balkans, la grande Camomille où Partenelle est aujourd'hui naturalisée sous les climats tempérés d'Europe et d'Amérique du Nord. Elle croît dans les décombres et les bords de chemins où contre les murs de maisons. D’abord introduite en Grande-Bretagne, puis en France, cette Asteraceae est légèrement poilue et reconnaissable à l'odeur caractéristique qu'elle dégage quand on la froisse.

La Partenelle est une herbe vivace de 30 cm à 50 cm de haut. Elle a des tiges vertes blanchâtres, d’abord couchées puis redressées et ramifiées dans leur partie supérieure. Elle porte des feuilles découpées, vert-jaunâtre, au bord crénelé, très aromatiques. Les fleurs sont réunies en capitules terminaux, composés, au centre, de fleurs tubuleuses jaunes et, en périphérie, de fleurs ligulées blanches. Les capitules sont, eux mêmes réunies en corymbes étalés. La floraison à lieu de mai-juin à septembre.

 La Grande Camomille est recommandée pour combattre les migraines, l'arthrite rhumatoïde et les douleurs menstruelles

Usage traditionnel

En médecine grecque, la grande Camomille est préconisée contre l'asthme et la mélancolie et la médecine arabe la revendique comme diurétique et emménagogue. Au Moyen Age, Hildegarde de Bingen l'emploie comme fébrifuge. Au XIXe siècle, elle est réputée stimulante, digestive, antispasmodique, emménagogue et fébrifuge. En cataplasme, elle soulage les douleurs de la tête, les migraines, les douleurs de la goutte et les névralgies faciales.

La Grande Camomille est particulièrement recommandée pour combattre les migraines, l'arthrite rhumatoïde et les douleurs menstruelles mais elle est déconseillée aux femmes enceintes.

L'histoire veut que l'intérêt scientifique pour la grande Camomille se soit manifesté après qu'un haut responsable des services de santé de la commission nationale anglaise du charbon (la National Coal Board), dont la femme souffrait de fortes migraines récurrentes, ait rencontré un mineur qui lui suggéra d'essayer les feuilles de grande Camomille, traitement traditionnel qu'il avait lui-même employé avec succès. À la suite de ce traitement, les migraines de la dame cessèrent presque complètement et le mari, intrigué et enthousiaste, en parla à un confrère, le docteur Stewart Johnson de la London Migraine Clinic. Ce dernier amorça une série d'essais cliniques qui contribuèrent à établir la réputation de la plante.

La Grande Camomille est aussi dotée de propriétés anti-inflammatoires et décongestionnante de la circulation sanguine.

C'est grâce au parthénolide, que les sommités fleuries de la Partenelle ont un effet antimigraineux reconnu. A la suite des recherches, le mécanisme de déclenchement de la migraine est dorénavant connu. Bien souvent les crises de migraines se déclenchent suite à un facteur déclenchant ; qu'il s'agisse d'un moment de stress ou d'émotion, de l'absorption d'un aliment ou même du cycle menstruel. Sous l'influence de ce facteur, de la sérotonine est libérée dans l'organisme. Sur les artères, la sérotonine produit une alternance de contractions et de dilatations des artères cérébrales à l'origine des douleurs. C'est alors que le parthénolide agit avec efficacité en bloquant la libération, par les plaquettes sanguines, de sérotonine responsable de la migraine.

Des études cliniques ont montré que la grande Camomille, en extraction alcoolique, diminue l’intensité de la douleur et réduit de façon importante la sévérité des symptômes généralement associés aux crises de migraine (nausées, vomissements, sensibilité au bruit et à la lumière).

La plante étant traditionnellement réputée pour son efficacité à déclencher les menstruations, il est conseillé de l'éviter chez les femmes enceintes.

Forme d'emploi et posologies.

• Infusion : 2 cuillerées à café de parties aériennes séchées pour une tasse (125 ml) d'eau porter à frémissement et laisser infuser durant 10 à 15 minutes. 1 à 2 tasses par jour.

• TM : 150 gouttes par jour dans de l'eau.

Migraine is a complex neurobiological disease characterized not only by headache, but also by nausea, photophobia (intolerance of bright light), phonophobia (intolerance of noise and speech), autonomic nasal symptoms, muscle pain, and cognitive disruption. Some attacks require bed rest, while others produce only mild or limited disability. Common acute treatments include acetaminophen/aspirin/caffeine combinations and over-the-counter nonsteroidal anti-inflammatory medications, as well as prescription products such as triptans and nonsteroidal anti-inflammatories. Because oral medications do not work for all migraine sufferers and because prescription medications are associated with adverse events, high costs, and quantity limits, there is a need for affordable, effective, alternative treatments.

The double-blind, placebo-controlled pilot study reviewed herein compared the efficacy of a unique lipid-based formulation of the sublingual homeopathic product LipiGesic™ M (PuraMed BioScience; Schofield, Wisconsin), which contains what the manufacturer states is a “3x”* preparation of feverfew (Tanacetum parthenium, Asteraceae) herb and a “2x”* preparation of ginger (Zingiber officinale, Zingiberaceae) root, (in a base of “Colloidal Silicon Dioxide, Natural Peppermint Flavor, N.F. Grade Olive Oil.” A peppermint (Mentha x piperita, Lamiaceae)-flavored sublingual preparation was used as a placebo. The trial tested the herbal-homeopathic preparation as an abortive treatment for acute episodic migraine. Secondary objectives included evaluation of persistent or recurrent headache 2 to 24 hours following treatment, assessment of adverse events, and comparison of migraine questionnaire scores between the treatment and placebo.

Sixty subjects enrolled in the study; data for 59 were included in the analysis. The subjects (46 women and 14 men) were aged 13 to 60 years, met criteria for International Headache Society migraine with or without aura, and had a history of migraine for more than 1 year. They had experienced between 2 to 6 attacks per month during the previous 3 months; at least 75% of their attacks had begun with mild headache. The subjects were able to differentiate migraine from non-migraine headache.

Conducted at 3 investigational sites, the study included 2 visits: one at screening and 1 exit visit 1 month later. The subjects were randomized 3:1 to receive either sublingual feverfew/ginger (n=45) or a matching placebo (n=15). The subjects were asked to treat all migraine attacks over the 1-month period, regardless of the intensity. They were instructed to treat with sequential administrations of 2 unit dose applicators. The liquid from 1 applicator was applied sublingually, held under the tongue for 60 seconds, and then swallowed. After 5 minutes, the second dose was administered. If headache pain persisted 1 hour later, the subjects could take a further 2 unit dose.

At screening, all subjects provided a medical, medication, and migraine history. Vital signs were recorded, and the subjects completed the Revised Patient Perception of Migraine Questionnaire (PPMQ-R). At the second visit, the subjects returned their completed headache diaries documenting the onset of headache pain and presence of migraine-associated symptoms, time of treatment with study medication, symptoms at 1 and 2 hours following treatment, time of relief, recurrence of symptoms within 24 hours after treatment, and adverse events.

Of those subjects using sublingual feverfew/ginger, 32% reported being pain-free at 2 hours post-dose compared with 16% using placebo (P=0.02). At 2 hours post-dose, the percentage of attacks rated as no pain or mild pain was 64% for sublingual feverfew/ginger compared with 39% for placebo (P=0.003). Pain level differences on a 4-point scale were -0.24 for those receiving feverfew/ginger and -0.04 for those using placebo (P=0.006). At 2 hours post-treatment, a statistically significant difference was noted in migraine symptoms and headache characteristics between the 2 groups. This included superiority of the treatment over placebo in eliminating the pulsating character of the headache (P=0.007); the worsening of headache with activity (P=0.015); and the presence of light sensitivity (P=0.001), sound-sensitivity (P=0.003), and nausea (P=0.02). The treatment was well tolerated.

Recurrence of headache was not analyzable because there were only 9 attacks in the placebo group that were pain-free at 2 hours after treatment. Of the 47 attacks that became pain-free at 2 hours in the active group, the recurrent rate was 20.4%, meaning return of moderate to severe migraine pain or use of rescue medication (i.e., conventional treatment) within 22 hours of becoming pain-free. Evaluation of the subjects’ responses on the PPMQ-R revealed statistical superiority for the treatment over placebo in total score (P=0.30), efficacy (P=0.04), and functionality (P=0.02).

According to the authors, the complex layers involved in the treatment of acute migraine are not addressed in clinical guidelines. Often, adverse events are interpreted by patients differently than by their healthcare providers. Consequently, an “adherence gap” exists between what providers think patients need for treatment and what patients seek from treatment. This is apparent, say the authors, in the fact that the use of triptans, considered the “gold standard” of migraine treatment, is not increasing by those with migraine headaches. Patients are seeking more than what triptans can provide.  “Understanding this ‘adherence gap’ represents the distance between evidence-based medicine and patient-centered care,” write the authors, calling for future studies to understand more completely the treatment dynamics of the migraine population.

The authors conclude that this study supports the efficacy of lipid-based sublingual feverfew/ginger in the acute treatment of migraine when administered early in the migraine attack.
