First calculate the patient's total daily requirement of IV morphine, which is the basal rate (0.5mg/hr x 24 hours = 12mg) plus + bolus dosage (28mg) = 40mg. Then multiply the IV or SC dosage by three to obtain daily oral dosage (40mg x 3 = 120mg). Finally, divide by 2 to get the proper 12-hour dose (120mg / 2 = 60mg).
