Fat Pad Syndrome

What is it? A condition where the inferior fat pad of the patella gets pinched between the patella and the femoral condyle. This can occur acutely with direct trauma to the knee or can be chronic with repetitive activity.

How does it present? Pain below the patella with activity, often worsened by prolonged standing or hyperextension.

Common exam findings: You may find tenderness and swelling of the fat pad below the patella. Patients may also have associated genu recurvatum, or excessive hyperextension of the knees.

Tests and treatment: No routine tests are needed. On magnetic resonance imaging (MRI) you may find some changes of the fat pad. Treatments can include ice, modification of activity, and a rehabilitation program. Taping techniques have been used as well.
