Osteitis Pubis Pubic Symphysitis

What is it: Inflammation of the symphysis pubis caused by repetitive forces across the pubic symphysis. Most commonly seen in runners and soccer players.

How does it present: Patients usually complain of gradual onset of exercise associated pain in the lower abdomen and medial thighs or groin region.

Common exam findings: Tenderness over the pubic symphysis can be appreciated. Pain may be elicited by resisted hip adduction as well.

Tests and treatment: Pelvic radiographs may be normal but can show widening of the pubic symphysis or evidence of sclerosis. A bone scan or MRI may help in diagnosis if x-rays are normal. Treatment often requires avoidance of aggravating activity and physical therapy may help. Improvement in symptoms may take several months.
