Heel Fat Pad Syndrome

What is it: Heel fat pad syndrome is heel pain in the middle of the sole of the heel, usually due to some contusive injury to the heel.

How does it present: Patients will often complain of pain in the middle of the heel. Plantar fasciitis usually presents with pain over the insertion of the plantar fascia at the front of the heel. Pain is usually described as deep and aching like a bruise and may have been precipitated by an acute trauma to the heel or repetitive trauma.

Common exam findings: The patient may experience tenderness over the middle of the heel.

Tests and treatment: Foot x-rays may be needed to rule out bony injuries. Treatment is generally conservative with rest, heel padding, and shoe modification.
