Hip Strain

What is it: Acute or overuse injury to the muscle groups around the hip. Commonly injured muscles include the hip flexors and adductors.

How does it present: Patients will usually complain of pain in the affected area that worsens with activity.

Common exam findings: Hip flexor strains are identified by pain with resisted hip flexion or passive hip extension. Thomas' Test may be used to evaluate for hip flexor contracture. Hip adductor strains are identified by groin pain with resisted hip adduction or passive hip abduction.

Tests and treatment: Radiologic studies are not necessarily needed for diagnosis, but in cases of acute onset of injury may be useful in ruling out possible associated avulsion injuries. Treatment usually includes avoidance of aggravating activity and conservative measures as well as physical therapy.
