Asymmetric removal of prepuce. See Figure 1 below.

In this image: Figure 1. This photo shows the asymmetrical removal of the prepuce.

To avoid this:

  • Remove all "adhesions" and visualize the entire corona.

  • Inspect for symmetric and adequate removal of foreskin above the bevel hole before tightening down the nut.

  • Excessive removal of the ventral prepuce. A common mistake.

To avoid this:

  • The Gomco base plate must be at an angle to the penile skin creases so that there is less ventral skin being removed than dorsal skin. See Figures 2 and 3 below.

In this image: Figure 2. The Gomco base plate must be at an angle to the penile skin creases.

In this image: Figure 3. This photo shows the Gomco base plate at an angle to the penile skin creases so that there is less ventral skin being removed than dorsal skin.

Reason for this:

  • Looking at lateral view of the penis demonstrates the angle of the corona to the penile shaft skin is between 45-60º and that there is less ventral glans than dorsal glans. See drawing in Figure 4 below.

In this image: Figure 4. An illustration of the lateral view of the penis.

  • Excessive ventral skin left ventrally. See Figure 5 below.

  • Not enough attention was given to proper alignment of base plate bevel hole so that insufficient ventral prepuce was removed.

In this image: Figure 5. Excessive ventral skin.
