Lateral Malleolar Fracture

What is it: A lateral malleolar fracture is a fracture of the distal fibula.

How does it present: Patient usually have some traumatic event or twisting of ankle and may present with difficulty with weight bearing, lateral ankle pain and swelling.

Common exam findings: Tenderness over the distal fibula and swelling and ecchymosis in this region. Difficulty with ambulating may be seen.

Tests and treatment: Ankle films are needed to confirm this diagnosis. An isolated lateral malleolar fracture without significant displacement or joint space involvement may be treated with splinting and casting for immobilization. Lateral malleolar fractures associated with ankle instability, other ankle fractures, significant ligamentous injuries, significant displacement of the fracture or intra-articular involvement may require surgical intervention.
