Help with Dose Conversions

The following method is recommended for converting oral morphine to an equivalent SC or IV dosage:

1. Calculate the patient's total daily requirement of oral morphine.

2. Divide that dosage by three to determine the patient's total daily requirement of SC morphine.

3. Divide the total daily requirement of SC morphine by 24 hours to obtain the hourly SC morphine equivalent.
