Syndesmotic Ankle Sprains

What is it: The syndesmotic ligament joins and stabilizes the tibia and fibula. This can be injured with inversion or eversion ankle injuries.

How does it present: Patients present with injuries of twisting, inverting or everting the ankle. Symptoms are similar to those on the typical ankle sprain, but may be more severe.

Common exam findings: Ankle swelling and general tenderness may be present. Kleiger's Test and the Squeeze Test may be positive.

Tests and treatment: X-rays may be indicated to rule out bony injury. Syndesmotic sprains are treated with supportive bracing or a short term of immobilization followed by functional rehabilitation. Symptoms may last longer than with the "typical" ankle sprain.
