Keep the base plate angulated upward as shown in Figure 1 below.

This helps prevent excessive pulling through of the ventral prepuce, because the ventral penis is shorter than the dorsal penis. See drawing below.

  • As you can see, the dorsal glans penis is longer than the ventral glans, giving an angle of the corona to the penile shaft of about 45-60°.

  • This implies that the excision of the prepuce should be less on the ventral side than on the dorsal side to prevent excessive ventral skin removal from the penile shaft, leading to a poor cosmetic result.

  • Pull up vertically on the dorsal hemostat until the apex of the dorsal slit is seen just above the bevel hole. Dab with a sterile gauze sponge as necessary. See Figure 2 below.
