Mitochondriopathie
Definition:
- Erkrankungen mit Defekt mitochondriale Energiestoffwechsel => Störungen oxidative Phoosphorylierung bzw. Substratgewinnung, mitochondriale Dehydrogenase bzw. Transportsysteme
- v.a. spontane Mutationen mitochondriales Genom, oft maternaler autosomal-rezessiver bzw. autosomal-dominanter Erbgang
Klassifikation:
- Skelettmuskulatur: mitochondriale Myopathie ( z.B. MELAS-Syndrom Myopathie, Enzephalopathie, Laktat-Azidose, Schlaganfällen)
- ZNS: mitochondriale Enzephalopathie (z.B. MERRF-Syndrom Enzephalopathie, Myoklonien, generalisierten Epilepsie => histologischer Befund “ragged red fibers”)
- andere Organe (z.B. Herz, Niere, hämatopoetisches Syndrom, etc.): mitochondriale Zytopathie
- biochemische Klassifikation: Defekte Lipidstoffwechsel / Atmungskette / oxidative Phosphorylierung / Pyruvatstoffwechsel / Zitronensäurezyklus / Malat-Aspartat-Shuttles
Klinik:
- Myopathie: Muskelschwäche (okulär, faziokapulphumeral) / Belastungsintoleranz / Myalgie / Kardiomyopathie / Ateminsuffizienz / De-Toni-Fanconi-Debré-Sequenz
- neonatale Form Enzephalomyopathie: Hypotonie / Apathie / Koma / Ateminsuffizienz / Laktatazidose
- infantile, juvenile bzw. adulte Form Enzephalomyopathie: psychomotorische Retardierung / Ataxie / periphere Neuropathie / Epilepsie / extrapyramidale Bewegungsstörung / Pseudohypoparathyreodismus / De-Toni-Fanconi-Debré-Sequenz
Diagnose:
- Anamnese / Laktat erhöht in Urin, Serum, Liquor) / BGA / Carnitin (Serum) / Muskelbiopsie (z.B. Trichomfärbung mit “ragged red fibers”) / Elektronenmikroskopie / Messung mitochondriale Enzymaktivität / Analyse mitochondriales Genom / Analyse Gene Lipidstoffwechsel
Therapie:
- symptomatisch / Gabe Coenzym Q (Steigerung oxidative Phosphorylierung)
