Myeloproliferatieve ziekte: polycythemia vera, essentiële trombocytose, myelofibrose
Solide of hematologische maligniteit
Paroxysmale nocturnale hemoglobinurie
Intra-abdominale infectie/inflammatie: IBD, pancreatitis, cholecystitis, abces, diverticulitis, appendicitis, cholangitis

Recente portatrombose: aanwezig <6 maanden
Chronische portatrombose: aanwezig >6 maanden
Occlusie
- Compleet: 100% van lumen
- Partieel: >50% van lumen
- Minimaal occlusief: <50% van lumen
- Caverneuze transformatie: macroscopische portoportale collaterale zonder dat v. porta geïdentificeerd kan worden
Respons op behandeling
- Progressief: toename grootte trombus of meer complete occlusie
- Stabiel: geen wijziging
- Regressief: afname grootte trombus of minder complete occlusie

Aanvullende diagnostiek

Geen aanvullende diagnostiek indien grote abdominale operatie of trauma, of significante intra-abdominale inflammatie
Overig
- JAK2 V617F mutatie (ook als bloedbeeld niet suggestief is)
- CALR mutatie indien JAK2 negatief
- Antifosfolipiden antistoffen
- PNH flow cytometrie (in principe alleen bij hemolyse of cytopenieën)
- (Geen indicatie erfelijke trombofilie)

Behandeling

Indien aanwijzingen darmischemie: antistolling
Geen cirrose
- Recente portatrombose (<6 maanden): antistolling ten minste 6 maanden
- Chronische portatrombose: antistolling indien trombofilie, uitbreiding in v. mesenteriaca of (eerdere) aanwijzingen voor darmischemie
Wel cirrose: case-by-case beslissing over antistolling
- Analyse high-risk oesofagusvarices voorafgaand aan start antistolling
- Recente partiële occlusie: overweeg behandelen
- Minimaal occlusieve trombose: overweeg monitoring iedere 3 maanden
- Progressieve trombus: behandelen
- Chronische complete occlusie of caverneuze transformatie: geen benefit antistolling
- Uitbreiding in v. mesenterica: behandelen
