Cardiopulmonaal: shock/sepsis, hypoxie, hypercapnie, hypertensieve encefalopathie
Neurologisch: meningitis, encefalitis, status epilepticus/post-ictaal, intracraniële bloeding, herseninfarct, massa-effect (tumor, abces, hydrocefalus), Wernicke encefalopathie, sinustrombose
Psychiatrisch: stil delier, conversie

Top-tot-teen lichamelijk onderzoek inclusief neurologisch onderzoek (asymmetrie, reflexen, corneareflex, pupilreacties, tonus, myoclonus, oogvolgbewegingen, fundi).

Laboratoriumonderzoek: bloedbeeld, CRP, natrium, kalium, kreatinine, ureum, calcium, albumine, leverenzymen, glucose, bloedgas (bij voorkeur arterieel, denk aan HbCO)
Op indicatie: ECG, fosfaat, magnesium, chloride, ammoniak, TSH, vitamine B1, vitamine B12, cortisol, toxscreen, bloedkweken, CT-hersenen (inclusief CT-A om basilaristrombose uit te sluiten en/of CT-V om sinustrombose op uit te sluiten indien coma), HbCO, lumbaalpunctie, PT, aPTT
Direct te overwegen behandelingen
Luchtwegmanagement: manoeuvres, Guedell/Mayo, intubatie
Hypoglycemie: geef glucose 10 gr = 100 ml van glucose 10% = 20 ml van glucose 50%, controle <10 minuten
Alcoholabusus: thiamine 500 mg iv
Sepsis/meningitis: antibiotica
Intoxicatie: antidota
