M. Crohn
- Man:vrouw ratio = 1:1
- Leeftijd: 20-40 jaar
- Remitting-relapsing patroon
- Complicaties: strictuur, fistel, abces
- Klassieke presentatie: diarree, buikpijn, gewichtsverlies, low-grade koorts, vermoeidheid, malnutritie
- Extrainestinale manifestaties: onder andere mond (aften), oog (uveitis/iritis/episcleritis), huid (erythema nodosum/pyoderma gangrenosum), gewrichten (artritis/arthropathie), galwegen (primair scleroserende cholangitis in 5%)
- Anamnese: klachten, buitenlandbezoek, gastrointestinale infecties, medicatie, roken (2x verhoogd risico), familieanamnese
- Lichamelijk onderzoek: o.a. inspectie peri-anale regio en rectaal toucher
- Laboratoriumonderzoek: bloedbeeld, CRP, kreatinine, albumine
- Aanvullend onderzoek (op indicatie): coeliakie, feceskweken, DFT, calprotectine (indien <40, dan 1% kans op M. Crohn), endoscopie

Colitis ulcerosa
- Epidemiologie: man:vrouw ratio = 1:1; diagnose vaak tussen 15-30 jaar, soms tussen 50-70 jaar
- Klachten: diarree (met bloedbijmenging), (loze) aandrang, buikpijn, koorts (in ernstige gevallen), extraintestinal manifestaties

- Remmissie-inductie gevolgd door onderhoudsbehandeling (zie stroomschema's onder)
- Doelen: mucosale genezing (geeft minder complicaties), kanker voorkomen, colectomie voorkomen, kwaliteit van leven
- Therapieën
- Budesonide: werkt enteraal, nauwelijks systemische opname, weinig bijwerkingen, werkzaamheid minder dan prednison
- Prednison
- Thiopurines (azathioprine en 6-mercaptopurine): met name indien steroid-refractair of bij steroid-afhankelijkheid
- Anti-TNFα: infliximab (altijd in combinatie met azathioprine om antistofvorming te voorkomen; -ximab = chimeer), adalimumab (minder effectief, maar minder risico antistofvorming; -mumab = 95% gehumaniseerd)
- Vedolizumab (remming α4β7-integrine waardoor remming migratie leukocyten naar darm; -zumab = gehumaniseerd)
- Ustekinumab (IL-23 remmer)
- JAK-remmers
- Vóór start anti-TNFα of thiopurine: EBV, CMV, hepatitis A/B/C/E, HIV, IGRA, X-thorax
- Chirurgie M. Crohn
- Acuut: persisterende ileus, bloeding die niet op een niet-chirurgische wijze te stelpen is, abces drainage niet toegankelijk voor percutane drainage, perianale abcessen.
- Electief: therapieresistente passageklachten, maligniteit en perianale fistels, eventueel refractaire ziekteactiviteit.
- Chirurgie colitis ulcerosa
- Acuut: therapieresistente toxische colitis of toxisch megacolon, perforatie, bloeding die niet op niet-chirurgische wijze te stelpen is.
- Electief: geen of onvolledige respons op optimale therapie, complicaties van medicatie, dysplasie of maligniteit, reconstructie na acute colectomie
