Including the word Doctor in the article’s title would be appropriate if it’s an integral part of a character’s name rather than a title (like Doctor Death), or if it’s genuinely a title but we don’t know the character’s given name (like Dr. Beerstein), so I would say to drop the Dr. from the title; however, there are plenty of counterexamples, like Dr. Watson, Dr. Jekyll, Dr. Martin Burke, Dr. Cliff Landon and Dr. Marcus Kemp.
