The spell teleport the caster to their designated sanctuary.
When the caster utters the word of recall, they are teleported to their designated sanctuary.
In addition to the caster's body, the caster can teleport up to 25-lbs. per caster level of gear or other material, including willing creatures. Anything worn or carried takes precedence to anything touched by the caster.
Only one sanctuary can be designated by the caster; Typically, this is the main altar in their own religious stronghold. Designation requires a special ritual (per the GM) to be performed. If no sanctuary has been designated, the caster will be transported to the main altar in a temple dedicated to their deity. The temple must be one they are very familiar with (where they completed their training, etc.).
This teleportation effect allows transport across planes.
The teleportation effect is always safe with no chance for errors.
Limitations
Unless ruled otherwise by the GM, a word of recall spell will fail if used from within an extra-dimensional space.
If the Astral plane cannot be accessed by the caster, the spell fails.
