The spell prepares an object that will teleport anyone breaking it.
When the spell is cast, the caster must specify a command word and a location. The location must be very familiar to the caster and if the spell is divine, the location must be the caster's designated sanctuary (cf. word of recall). The spell is cast into a specially prepared object (the succor item—see Material Components). Anyone that breaks the item while speaking the command word will teleport to the specified location.
Casting Time. The casting time is a ritual lasting at least 24 hours and could be longer for the non-divine version of the spell. The non-divine version can be cast a maximum of once per month. Any break in the ritual destroys the component and the spell fails.
Duration. The item retains its magic until used or the magic is dispelled.
Break Item. (major) The possessor breaks the succor item while verbalizing the command word. The possessor is then teleported to the location specified by the spell.
In addition to the possessor's body, the possessor can teleport up to 25-lbs. per caster level of gear or other material, including willing creatures. Anything worn or carried takes precedence to anything touched by the possessor.
This teleportation effect allows transport across planes. If the spell is divine, the teleportation effect is always safe with no chance for errors. Otherwise, there is a 1% chance per plane ("# planes removed" from the location as per Planes of Existence) that the possessor and everything carried is lost forever (only an avatar-level wish effect can retrieve them).
Limitations
Unless ruled otherwise by the GM, a succor effect will fail if the item is broken from within an extra-dimensional space (and a broken item cannot be used again).
If the Astral plane cannot be accessed by the at the time the succor item is broken, the spell fails (and a broken item cannot be used again).
Material Components
An easily breakable object. For the divine version, this is usually a religious item (prayer beads, etc.). Cleric Version. Holy relics and rare religious materials. Magic-user Version. A large gem. Cost/Scarcity. Cleric Version. 1d4+1 x 1,000 gp. Magic-user Version. At least 5,000 gp./Very Rare.

Break Item. (major) The possessor breaks the succor item while verbalizing the command word. The caster is then teleported to the location the item was broken (without delay and without an preparation on the part of the caster). Cleric Version. The caster can choose to ignore the summons (and the item is broken and the spell "fails"). The caster only knows which item has been broken but no other detail. Magic-user Version. The caster cannot choose to ignore the summons.
In addition to the caster's body, up to 25-lbs. per caster level of any gear carried at the time the item is broken will be transported (this effect includes a familiar if the familiar is being carried).
This teleportation effect allows transport across planes. If the spell is divine, the teleportation effect is always safe with no chance for errors. Otherwise, there is a 1% chance per plane ("# planes removed" from the location as per Planes of Existence) that the caster and everything carried is lost forever (only an avatar-level wish effect can retrieve them).
Limitations
Unless ruled otherwise by the GM, a summons effect will fail if the item is broken from within an extra-dimensional space (and a broken item cannot be used again).
If the Astral plane cannot be accessed by the possessor and/or the caster at the time the summons item is broken, the spell fails (and a broken item cannot be used again).
