Save vs. Spell (save negates)
The spell moves the recipient instantaneously to another plane.
When touched, the recipient is physically relocated another plane of existence along with all their gear and any objects carried. Objects must be off the ground and unconnected to anything else; otherwise the object will not be shifted with the creature.
If the recipient is willing and is touching another willing creature with at least two points of contact (e.g. holding both hands), then more than one creature can be shifted. Up to six creatures other than the recipient can be plane shifted this way, if all are using two limbs to touch each other (in a circle). The caster can position themselves so as to be part of the circle.
An unwilling creature must be successfully touched using a contact attack and must make a save vs. Spell or they will be shifted to the plane selected by the caster (save negates). More than one unwilling creature cannot be plane shifted (unless the recipient is carrying a familiar, in which case, the familiar will also be shifted).
Other than the strictures above, there is no weight limit for the spell.
When the caster completes the spell, if the material component was used correctly, the recipient(s) will be shifted bodily to the desired plane. The caster will also visualize a spot within the plane to shift the recipient. This spot must be at least researched by the caster, otherwise, if no knowledge exists, the spot will be totally at random. Even if the spot is well known, the recipient(s) likely won't find themselves right "on target" but will be nearby (at least in planar terms). Random placement typically places the recipient(s) on top of solid matter unless none is in the area.
When shifting from any other plane to the caster's originating plane (where they were born or incepted), the desired location can be targeted exactly but it must be familiar to the spell caster (previously personally visited) or random placement will be determined as above. Thus, two plane shift spells can be used as a teleport spell of sorts if the intended destination is familiar to the caster and on their originating plane.
Limitations
Unless ruled otherwise by the GM, a plane shift spell will fail if used from within an extra-dimensional space.
If the Astral plane cannot be accessed by the caster, the spell cannot shift recipients to an Outer Plane.
If the Ethereal plane cannot be accessed by the caster, the spell cannot shift recipients to an Inner Plane.
While the Astral Plane, Ethereal Plane, or Plane of Shadow can be chosen as a destination, unless the caster has been to a specific location that plane (personally), their location will be totally at random within the plane (and always within the realm of a powerful being).
Material Components. A tuning fork or forks. Each plane requires the turning fork to be made of a different metal and tuned to a specific note and pitch. If the plane is multi-layer, only the top layer can be accessed unless multiple forks are used to produce chords. The information regarding which type accesses which plane is jealously guarded and recipes to access sub-layers are often the product of a specific quest. Powerful beings that control the fabric of a plane can change the requirements for access at any time (and do so frequently). The tuning fork is not consumed by the spell. Cost/Scarcity. Varies by material/rare. The fork must be made by a musical instrument maker with the aid of a smith (1d6 days; 25 gp in service expense).
