It looks like slovenian clubs change their names often. In fact, it is sponsor names that change while team names usually remain. I'll focus on real team names and on clubs that no longer exist but have reestablished clubs with same name.

NK Ljubljana (formed 1909). Also known as Hermes and Železničar. Dissolved 2005. Not to be confused with FC Ljubljana (Formed 2005 , dissolved 2011). NK Slovan – Also known as Slavija Slovan. NK Beltinci (formed 1949) Also known as Potrošnik. Dissolved in 2006. Not to be confused with NK Beltinci (formed 2006). NK Nafta (formed 1903) – dissolved in 2012. Not to be confused with NK Nafta 1903 (formed 2012). NK Steklar – Dissolved in 1999. NK Primorje (formed 1924) – Dissolved in 2011. Not to be confused with ND Primorje (formed 2011). NK Domžale – Also known as Napredek. NK Železničar Maribor – Dissolved in 2019. NK Korotan Prevalje (formed 1933) – Dissolved in 2003. Not to be confused with DNŠ Korotan (formed 2002). NK Kovinar Maribor – Dissolved in 2006. NK Partizan Slovenj Gradec – Dissolved in 1995. NK Krka – Also known as Elan Novo Mesto. NK Slavija Vevče – Also known as Set Vevče. Dissolved in 1999 after being swallowed by Slovan. NK Piran – Also known as Solinar. Dissolved in 1988. NK Šmartno (Formed 1928) – Dissolved in 2005. Not to be confused with NK Šmartno 1928 (formed 2005), that is other club. NK Radomlje – Also known as Napredek Radomlje. NK Jadran – Represents both villages of Hrpelje and Kozina. NK Renče – Also known as Goriške opekarne. Dissolved in 2002 after being swallowed by Brda Dobrovo. NK Drava Ptuj (formed 1933) – Dissolved in 2011. Not to be confused with NK Drava (formed 2004), that was B team of NK Drava(formed 1933) and plays today as its own entity. NK Mengeš – Dissolved in 1997 NK Šentjur – Dissolved in 2000, not to be confused with NK Mladi Upi, that is other club. NK Črnuče – Dissolved in 1998 after being swallowed by Factor Ljubljana. NK Ankaran Hrvatini – Dissolved in 2019. NK Interblock – Also known as Factor Črnuče and Factor. Currently inactive. Nk Bela Krajina – Dissolved in 2016. NK Grosuplje – Also known as Tabor Grosuplje. Dissolved in 2003. NK Zavrč – Dissolved in 2017. NK MURA 05 (Formed 2005) – Dissolved in 2013. Not to be confused with NK Mura (formed 1924, dissolved 2005) and NŠ Mura (Formed 2013). NK BONIFIKA – Dissolved in 2010. Club was originaly from Koper, later relocated to Izola.
