To close an application, you need to submit a request to voluntarily withdraw the application for licensure. This workflow is available for resource workers and LCPA workers.
From the main page of a Foster Family resource that is in the Initial Application Pending state, scroll to the Recommendation and Request card.
Click Add, and select Voluntary Withdrawal of Application
In the popup that appears, select all the reasons the application is being voluntarily withdrawn, enter the closure date, and click save.
Several changes will be made on the main overview page:
The Recommendation and Requests card will update (If you click the ‘Delete Recommendation’ link, a popup will appear asking “Are you sure you would like to delete this recommendation/request for Voluntary Withdrawal of Application?”)
New notification bars will display at the top of the page
Click "Submit for Approval" at the bottom of the page. This will send the request for supervisor approval.
The notifications at the top of the page will update. Any comments to the supervisor can be found on the history page.
The License Summary card will also update with the license status: Initial Application Pending, Supervisor Review, Application Withdrawn Voluntarily.
When the supervisor approves the request, the application will be withdrawn. The supervisor can also return the request for edits.

There are a number of reasons a licensed resource may have its license withdrawn voluntarily. The list of options for voluntary withdrawal in Casebook include:

The voluntary withdrawal workflow is started from the Recommendation & Requests card on the overview page of a licensed resource. Before any recommendations or requests are added to this card, you will see a message explaining that the functionality of the card is currently limited. You will also see a link that will take you to the edit page of the License Summary card, on which the recommendation for initial licensure can be entered.
When you submit a request to withdraw a license, any open, incomplete revision checklists on the resource are ignored. These checklists, and any related files like home studies, do not need to be completed or uploaded before submitting the voluntary withdrawal.
Entering a request for voluntary withdrawal of a license
On the overview page of a licensed resource, go to the Recommendations & Requests card.
Click Add and select Withdraw License (Voluntary).
A popup box will open. Select all the reasons for withdrawal, and enter the date the withdrawal will be effective form.
Click save.
This will trigger the progress bars at the top of the overview page to change. They will look like:
The Recommendation & Request card will also update with the reason, the effective date, and the related file when it is uploaded (as explained below).
You will also need to upload a Voluntary Withdrawal form to the file uploads card. This can be done before or after the previous 4 steps; if the Voluntary Withdrawal form is not uploaded and marked a “Voluntary Removal Form” you will receive an error validation asking them to do so.
To upload the form,
Scroll to the File Uploads card on the overview page of the license and click Edit/Add
Click Upload File
Select the Voluntary Withdrawal form from your computer. Click add in the popup box that opens.
Click the edit link associated with the file you just uploaded.
Select Voluntary Withdrawal form as the file type and enter a date.
Click Save.
Click Close.
You will be taken back to the overview page.
Click the Submit for Approval button at the bottom of the overview page.
Enter the name of the supervisor you want to review the request for voluntary withdrawal and any additional comments in the popup that opens
The resource will need to go to your supervisor for review, but will not need to go to Central Office for review.
A workflow event card will be added to the history page for this event.
The progress bar will change slightly to highlight that the resource is in supervisor review
The supervisor will review the resource and either send the resource back to you for edits or approve the voluntary withdrawal.
If approved:
The progress bars will change
The License Summary card will change
The Recommendation & Requests card will change
NOTE: As you're working through the Voluntary withdrawal of license workflow, a new workflow event card will be added to the history page as you submit each step. Any comments on the submissions will be displayed on the related workflow event history page card.
