When an FCM views an assessment they will see the victim, the perpetrator, the allegations, and the details of the Intake Report. FCMs can then complete mandatory contacts, upload evidence, create safety and risk assessments. The necessary data elements that need to be completed to submit an assessment are:

Consent to interview must be obtained for all children in the assessment
All mandated contacts must have been contacted face-to-face. This is what initiates the assessment
Workers can create an exception for any of these required pieces of information by clicking on the small red exclamation point located next to the piece of information on the assessment overview page. To learn more about creating an exception, click here.
Once all necessary information has been entered into the assessment, the FCM submits the assessment to their supervisor.
Click Submit at the bottom of the Assessment.
A popup will open. Select the supervisor you want to submit the assessment to and enter any comments.
The supervisor will review the Assessment and either accept it or return it for edits.
When an Assessment has been approved, the supervisor will see a popup that asks whether they are sure they want to approve the assessment.
After the assessment has been approved, the notes attached to it will no longer be editable
When an assessment has been returned by the supervisor for edits, three things will signal this to the worker:
The worker will receive a notification in Casebook
There will be a note on the Assessment's card on your dashboard
Beneath the chevron progress bar, there will be a notification bar stating the assessment has been returned for edits with the date and the supervisor's name.
There will also be a link to the comments. You can hover over this link to preview the comments or click it to be taken to the history page. The history page will be filtered for workflow events and will be where the full comments are stored.
When you submit an assessment for approval, you will be alerted if there are focus children in an open Removal / Placement that are not associated with an open Case.
A two part error message will display:
Placements must be ended, or there must be an active case for [child]
Open Removals must be ended, or there must be an active case for [child]
You can close the removal episode or create a case with the child victim who is in the open removal episode as the focus child. Either action will satisfy the requirement and you will be able to submit the assessment.
If the placement on the assessment is ended, but the removal episode is not also end dated, when you try to submit the assessment, you will receive an error message stating the “Open Removals must be ended, or there much be an active case for [child].” Only once the placement and removal episode has been ended or a case has been created with the victim as a focus child can the assessment be closed.
