The 3CM role will be managing Collaborative Care (CC) cases and, if necessary, entering and uploading the information CEU workers will use to make determinations on the eligibility files for older youth.

Collaborative Care eligibility applications are created for children in Casebook when a child enters an initial placement and he/she is 18 and older.
The CC eligibility applications will be processed by CEU workers. But, a lot of information the CEU workers will check to make the determination will be entered by the 3CM.
Collaborative Care worker can now upload Voluntary Placement Agreements to a case and tag them as such. These agreements will display on the Collaborative Care eligibility application in the Court Hearing section of the Manual Initial Criteria card.
Go to an active case that is associated with Collaborative Care.
Scroll down to the Upload Files card and click the Edit/Add link in the top right corner.
Click Upload File and select the file from your computer you want to upload.
Click the edit associated with the file you just uploaded.
For the document to be counted as a Voluntary Placement Agreement in other places in Casebook, you must:
Select Voluntary Placement Agreement as the document type
Add a date
Add an associated person using the autocompleter
If you try to save without filling in these fields, you will receive an error message that reads “Attachment could not be updated”
Once saved, workers will get a success message
When a focus child enters the Collaborative Care program, a 3CM must fill out earned income, unearned income, assets, and expenses for the focus child and any of the focus child’s children. A warning message will appear on the Employment and Finances card on the Person Profile Page of everyone the 3CM needs to enter information for. Once income information has been entered for each type of income, the warning bubble will disappear.
The 3CM assigned to the focus child's case will also receive a notification to enter income information for any of the focus child's children, if applicable, for the month of the eligibility triggering event. If a Collaborative Care removal episode exists already, and a child relationship is added so the focus person is now a parent, Casebook will create an income record requirement for the focus child's child for the month of the start of the removal. 3CM workers will be notified that they need to enter eligibility month income for this person.
There is now a new involvement type of “3CM Collaborative Care/CHINS”. This will be used by 3CMs for children who are receiving services from the Collaborative Care program.
To change the involvement type, click edit on the Focus Child care on the Case page. More detailed information about how to edit involvement types can be found here.
3CMs can create a court hearing called “Collaborative Care Hearing.” This court hearing is created through the same workflow as all other court hearings. Read more about creating court hearings here.

Dismissal of Collaborative Care Petition
Hearing Not Held
Hearing Reset/Continuance
