Collaborative Care (CC) is an eligibility program for individuals aged 18-20; a child can be eligible from the day of their 18th birthday through the last day of the month of their 20th birthday. The program only applies to child who have turned 18 after July 1, 2012.
All youth with the involvement types of Collaborative Care or 3CM Collaborative Care/CHINS, and youth 18 years old or older as a removed CHINS, can continue to be IV-E eligible if they meet special criteria at age 18 and 19 until the youth reaches the age of 20.
When a youth who is already in care is approximately 17.5 years old, an FCM or Collaborative Care Case Manager (3CM) will begin discussing the Collaborative Care program with them. If the youth is interested in the program:
s/he will be transferred from the FCM to a 3CM
The involvement type will be changed to 3CM Collaborative Care/CHINS.
The youth will remain in their current placement, their removal episode will not be closed and their eligibility will remain unchanged. At the time that responsibility for the youth transitions to a 3CM, the CHINS involvement (not the case) should be closed, and a new case should be opened in MaGIK.

At the age of 20 or prior to, the 3CM would close the case through court, enter the appropriate hearing and court outcome. In Casebook, the CC application will automatically close on the older youth's 20th birthday.
JD/JS youth will only be moved into the Collaborative Care Involvement type after their JD/JS involvement type is closed and will be managed by a 3CM, not a probation officer.
Youth that leave care and custody the month before their 18th birthday or after, can return to DCS and be a part of the Collaborative Care program. The youth must have been in foster care the month prior to their 18th birthday and fulfill the other Collaborative Care eligibility criteria to be eligible to receive CC assistance.
The youth in the CC program can be placed in:
Host Homes -- Can be found on the list of Placement Providers or Foster Family Homes
Supervising Agencies -- Can be found on the list of Placement Providers
Users should not be creating new Supervising Agencies. These will be created all at one time once the contracts have been set in place. If a Supervising Agency is not found under Placement Providers, Alishea Hawkins should be contacted. The Supervising Agency is used when the youth is placed in an apartment, dorm, etc.
A Collaborative Care eligibility application will be created for an older youth when a new removal episode is created for a child between the age of 18-20.
Once a new Collaborative Care eligibility application is created, it will appear in the Eligibility File Index where it can be assigned by a CEU worker. Assigning CC eligibility applications is done in the same was as assigning all other eligibility application. To read and learn more about the assignment process, click here.
CEU workers can filter the index specifically for CC applications; by default, however, the eligibility file index is filtered to display only EA and IV-E applications.
CEU workers can also sort the index by number of days since event date for CC applications.
