To relicense a resource whose license has been withdrawn, but whose initial license period has not yet expired, you can reinstate the resource's license. This is only an option for resources who have voluntarily closed their license during the initial license period.
Go to a resource whose license has been withdrawn.
Scroll to the Recommendations and Requests card. You will see a call to action asking you to add a Reinstatement License Request. Click the link in the call to action or the add link in the top corner to add the Reinstatement request.
Once the request has been added
a reinstatement checklist will be added to the Checklist card and the Recommendation and Request card.
The chevron progress bar will change to Reinstatement Pending at the top of the main page of the resource.
To delete the request, click the Delete Request link in the top corner of the card. This will close the reinstatement checklist; it will not be deleted from the resource. The chevron progress bar will also be removed and the license will return to a withdrawn state.
Complete the reinstatement checklist to continue along the reinstatement workflow.
Submit the reinstatement request for approval by clicking the button at the bottom of the page.
Select the supervisor you want to submit the request for approval to and enter any comments you want to send along with the submission. These comments will be stored on the history page of the resource.
The chevron progress bar at the top of the page will move to supervisor review. The supervisor can either accept the request or return it for edits.
If accepted, the license will return to an effective state, with the same end date as it had before it was withdrawn.
There will also be an approved request listed in the bottom section of the Recommendations and Requests card.
Reinstating a withdrawn Foster Family Home license will now activate any checklists that were active prior to the license being withdrawn
After the license has been reinstated, using the steps above, if there were open revisions on the resource, the status bar should read "Pending Request: Reinstatement (with Revisions)"
A revision slug for each revision checklist that was active when the license was withdrawn
Reverting changes via the revert button or directly changing the information should remove the associated revision slug and checklist.
Any revision change checklist that was active before the withdrawal and has not been reverted should be required upon submission for reinstatement.
If the change in address checklist was active prior to the withdrawal and the revision has not been reverted, the home study should be required upon submission for reinstatement.
