A pre-adopt plan is created by the FCM once they have identified a possible foster family for a child.
The work flow is completed in partnership with CEU.
The FCM will create the plan. Once the FCM saves the plan it populates the Adoption Eligibility Index and CEU workers will receive a notification.
The new plan for adoption will move to the top of the Adoption eligibility file index and a CEU worker will make a determination on it.
Once the determination has been made, the assigned FCM will receive a notification. The FCM will then finalize the plan with the family and send it back to CEU.
CEU will then officially finalize the plan for adoption.
From the main page of a case, scroll to the Plans and Tools card and click Add.
Select Pre-Adoption Plan
A checklist form will open.
Fill out all necessary information. Some questions will prompt you to enter additional information after answering, these include:
When selecting the pre-adoptive family the autopopulater will display in- and out-of-state resources.
Regardless of the type of resource selected (even if the resource is Out of State), the race and ethnicity selected on the individual family member's profile pages will automatically update on the plan. This information must be entered by the FCM on each person's profile page before the pre-adopt plan can be submitted; the pre-adopt plan can be saved without this information.
When answering Yes to the question "Does the child have a document diagnosis of a mental, emotional, medical or physical disability?", a modal will open with a checklist pre-populated with the medical conditions listed on the Health card of the person profile page. There is also link to the child's profile page to help the FCM enter any medical conditions.
When answering Yes to the question "Was there a voluntary or involuntary Termination of Parental Rights for the parent(s)?", date boxes will appear for each parent. You can fill in each date when applicable.
At the bottom of the form, you will see two save buttons: Save & Close and Save & View. At any time, you can fill in some information, click one of the save buttons, and leave the form to return to it at a later time. After information has been entered and saved to the form, any unanswered questions will have 'Not yet answered' beneath them.
Clicking Save & Close will save your information and return you to the main case page.
Clicking Save & View will save your information and bring you to an overview/edit screen for the Pre-Adopt plan.
To edit an incomplete Pre-Adopt plan, click the linked name in the Plans and Tools card on the main case page.
From the edit screen, you will see the information from the Pre-Adopt plan separated into cards. You will also see a progress bar across the top, letting you know the state of the Pre-Adopt plan. The state of the plan will also be listed in the Quick Info menu on the right hand side of the page.
Click the edit link in the top right hand corner of a card to be taken directly to those questions on the editable Pre-Adopt form.
The Pre-Adopt plan will populate the Adoption Eligibility Index. CEU workers can navigate here through the Resources & Lists menu in the Universal Toolbar.
The new pre-adopt plan will be at the top of the index. The index lists the focus child's name, the assigned worker, the type of application, and the number of days it has been in the queue. CEU workers can sort the index by clicking on the column headers.
CEU should click on the child's name of the pre adopt plan they want to make a determination on. This will open the edit page for the plan. CEU workers will see the answers the FCM added to the plan and fill out the information on the Eligibility and Subsidy card at the bottom of the page.
Click Edit to add information to the plan
Any questions that have not been answered will have a "Not Yet Answered" message below the question.
After the CEU worker has completed the determination and saves it, a popup box appears asking the CEU worker if they want to alert the assigned FCM the determination is complete. The CEU worker should click ok.
The FCM should complete the application with the family offline.
Once finalized, open the pre-adopt plan and click submit to send it back to the CEU worker
The pre-adopt plan is sent back to CEU for review. The assigned CEU worker will get a notification that the plan is ready for review.
Upon review, CEU can accept or return the plan for edits.
After the CEU worker has accepted the application, the FCM will receive an email and in-app notification.
If CEU rejects the application, s/he can enter a reason for the rejection that will be sent to the FCM via an email and in-app notification.
Sometimes after CEU makes a determination, the FCM will find that they can't get the family enough money to make adoption financially viable for the (otherwise-interested) family and the FCM will need to close the inquiry.
Open the "Resources & Lists" menu and select "Adoption Eligibility Application"
Select the pre-adopt plan to close.
At the bottom of the screen, the FCM will see a Close Inquiry button. Clicking this will open a box on the screen asking workers if they are sure they wish to close.
After clicking OK on the popup box notification, the FCM will be taken back to the linked Case Plan and a record of the closed adoption application. Clicking on this record will show workers an uneditable view of the pre-adoption plan.
