When you create a new removal episode, you will also need to complete the AFCARS removal questionnaire for every child involved in the removal episode. To remind you, a warning message and link, will appear on the Placements and Locations card on the case or assessment.
This questionnaire must be completed and saved before the removal episode can officially be ended with a court hearing.
To complete the AFCARS questionnaire from the Placements and Locations card on the case or assessment, click the (X of 7 questions answered) link after the Focus Child and removal
Clicking this link opens the questionnaire. Answer all of the questions using the radio buttons. Once all questions are answered, enter approving supervisor and the approval date then click Save.
Clarifying tips for the questions on the removal questionnaire
The first question: asks about the timing of the court outcome.
The third question: asks about the structure of the family from which the child was removed.
The fifth question ("Did any of the following conditions lead to this removal?"): you can mark all questions as yes or no, but at least one response needs to be marked yes before the questionnaire is submitted. You should fill in this section as accurately as possible.
There are three indicators in Casebook to signal you to complete the removal questionnaire for each removal episode.
A slug appears on the Placements and Locations card on the main page of the case or assessment. This slug will be present until you have completed the questionnaire. Each removal questionnaire required is listed within the slug along with the number of questions that are needed to complete them.
The amount of questions answered and a link to the AFCARS Questionnaire is found at the top of the Location History page. Another link to the AFCARS Questionnaire can be found at the beginning of each removal episode for the Focus Child/Victim. The link at the beginning of the removal episode will always be displayed.
Completing the Questionnaire
When the questionnaire is complete, two things will happen.
At the top of the page will be a message, " You have successfully completed an AFCARS Removal Questionnaire for 'Child'." A link to the completed questionnaire appears at the top of the Location History page. Clicking this link allows workers to edit their responses to the AFCARS questionnaire.
The warning message on the overview page of the card will no longer show.
If you try to save a removal questionnaire without entering all required information, you will see several error messages
A red outlined box will appear at the top of the screen calling out all of the missing information.
A red pill error message will popup
Inline error messages will appear next to any fields with missing information
