A 311 form is a summary of the assessment. FCMs are legally required to provide this form to various parties once the assessment has been completed. At the bottom of this page, you can download a template that illustrates the correct format of the 311 form.
The 311 form can be printed from the DCS forms portal, which is accessed from the Forms menu in the smart bar on an assessment.
Go to an assessment
From the assessment view page, open the Forms menu in the Smart Bar.
Click "All DCS Forms" to Launch the DCS Forms Portal. From there you will be able to print the 311 form.
The 311 form should be uploaded to the Assessment at the time that the assessment is approved to save an "as of approval" version of the 311.
The 311 was converted to a PDF document and uploaded to the converted historical assessment.
There is an edit that blocks the production of a new 311 from DCS forms for historical assessments as the PDF is the 311.
When an historical assessment has been edited due to appeal, the edit will no longer block the production of a 311 from DCS forms.
The historical 311 PDF will remain uploaded to the assessment but a new 311 can be produced and should be uploaded (or if redacted, scanned) into the assessment.
The new 311 will display in the revised 311 format so there could be differences in addition to the changed findings and post-assessment information and new approval dates that will print out on the form. New rules for populating fields using the family network may result in people appearing that were not on the original 311. At this time the solution is manual redaction. If the assessment was originally approved prior to the revision of the conclusion narrative into different categories, the conclusion narrative will not print out.
