The Caregiver Strengths and Needs Assessment identifies the primary strengths and needs of the family and youth, which is then used to develop an appropriate service plan and to address the needs of the caregiver(s) during the Child and Family Team Meeting. The Caregiver Strengths and Needs Assessment can only be created from a Case Page.
The tool includes a 3-page flow, marked by a progress bar at the top of the page. The three pages are:

From the Plans and Tools card on a Case page, click Add
Select Caregiver Strengths and Needs Assessment
You can use the back button on the browser to return to the Case page. If there is unsaved information on the page, workers will receive a warning asking if s/he wants to leave the page. If workers click leave page, all unsaved information will be lost.
You do not need to complete the assessment in one sitting. You will see three "save and" buttons at the bottom of the assessment edit pages. Clicking Save and Proceed will allow you to save the answers on the current page and move to the next page in the assessment. Clicking Save and Close will save the answers on the current page and take you back to the overview page of the case.
Once you are on the overview page of the case, you can edit the assessment by clicking on the linked assessment name on the Plans and Tools card.
The overview screen of the assessment will open. Each page of the assessment will be listed in its own card. You can edit each page of the assessment by clicking the edit link in the related card. Once all information has been entered, you can submit the assessment. All required fields are marked with red asterisks.
Upon completion of the Caregiver Strengths and Needs Assessment, a pop-up box will appear asking the worker to confirm that everything on the assessment has been completed. If so, workers can click Submit.
If there are unanswered, required fields on the assessment, the standard 3 error messages will appear.
A red outlined message will appear at the top of the page that lists which fields are missing from the assessment.
A red pill message will popup at the top of the page.
Inline warning messages will appear next to the unanswered required fields.
Workers have 7 days to edit the assessment once it has been submitted.
Deleting Caregiver Strengths and Needs Assessment can only be done if the plan has not already been approved. You should only delete a plan or tool if it has been created in error. This is not the correct workflow for ending the plan or tool.
After creating and saving the Caregiver Strengths and Needs Assessment, scroll to the Plans and Tools card. Click on the link for the Caregiver Strengths and Needs Assessment listed to open the edit screen.
On the view page, go to the Action dropdown and select delete.
A confirmation box should appear that asks “Are you sure?” Click ok to confirm the deletion. The Caregiver Strengths and Needs Assessment will no longer appears in the Plans and Tools card.
