The overview page for an eligibility application is populated by menus on the left and right side and cards in the center of the page, which store relevant information to the file. The screen shot below gives a partial view of the overview page of a Collaborative Care (CC) eligibility application.
Only CEU Supervisors, CEU workers, Admin, and Super Admin worker roles can edit eligibility applications. If you do not have one of these roles assigned to you in Casebook, you will see a yellow banner at the top of the application explaining that you are unable to edit the application.
Jump Menu: This menu is located on the left side of the eligibility application and follows you as you scroll up and down the page. It is populated with links that correspond to every card on the eligibility file. When a link is clicked the worker is taken directly to that card.
Actions Menu: This menu is located on the right side of the eligibility file. You can click to open a dropdown menu from which you can print the Medicaid Eligibility Form and add IV-E Waiver.
Application Details: This menu is located on the right side of the eligibility file below the Actions menu. It lists
demographic information about the child
a link to the focus child's person profile page
a link to the related unit of work on which the child's removal occurred
a link to the related FCM's user page and contact information for the FCM
a link to the assigned CEU's user page and contact information for the CEU worker
All Applications Menu: This menu is located beneath the Applications Details menu and lists all eligibility applications related to the focus child. The application you are currently viewing is listed in black.
The center section of the application has two page cards:
Application: Displays information about the type of application, the percentage complete, the initial determination and the initiating event.
Manual Initial Criteria: A checklist that lists the initial criteria a child must pass in order to be considered Eligible. This card also contains information about the child’s specified relative and related court hearings.
Select whether or not the child is a US citizen.
Select whether or not the child is under 20 years old
First select the Assistance Group. On a Collaborative Care application, the assistance group alway defaults to only the focus child.
Expand the Household Members dropdown at the bottom of the Income and Assets section of the Manual Initial Criteria card. The name of each person listed in the menu is linked to their person page and their relationship with the focus child is listed in parenthesis.
You will also see if any of the household members are in removal episode themselves in the event month; multiple removal episodes will populate as well. The start date of the removal episode will be listed and the link will take you directly to the edit screen of the Removals and Locations card for that person so you can see more details about the removal.
All household members are possible assistance group members. To determine which household members should be in the assistance group, look in the following places.
On the focus child's Person Profile Page. Click the edit link on the Family Network card to find information about the focus child's relationships and households. Clicking View all Household history will help you see who lived in the household with the focus child and when each person was member.
On the Profile Pages of household members. When you are viewing Household tab of the edit screen on the Family Network card, each member's name is linked to their person profile page.
Toggle back to the eligibility application and use this information to select the correct assistance group. Members can be selected in/out using the check boxes on the Specified Relative section.
For CC applications, the focus child will be their own specified relative.
There are two sections for court hearing information on the CC application.
Court Hearings & Agreements: this section relates to whether the child has a Voluntary Placement Agreement in place.
Court Hearings & Voluntary Placement Agreements: this section relates to whether specific court language has been appropriately found in the court documents uploaded by the FCM.
In this section of the card, all of the child's related court hearings, related outcomes, and voluntary agreements are listed in an expandable menu. Clicking the blue "All Court Hearings.." heading will expand/collapse the menu. In the menu, you will see each court hearing listed with the associated date, name (which is linked to more detailed information about the court hearing), and then two check boxes so you can indicate if either type of court language was found in the documentation related to that hearing.
If a court hearing name has a small paper clip icon next to it, it means there are documents attached to that court hearing.
This section of the manual initial criteria card asks if the child is meeting the education and employment requirements. Both questions will be constantly listed, however they will only be enabled when the child fits the appropriate age criteria.
If these questions are answered Yes, the child is fully reimbursable for a year.
If these questions are not answered, the child is considered eligible for that time period.
At the bottom of the application, you will see two buttons: Save and Submit.
Save: at any time you can save the work you've done on the application, close it, and return to it at a later date. Each time save is clicked, the percentage complete, in the Application card will update.
Submit: this submits the application, which sends information to KidTraks and begins back end calculation to verify the information on the application. This button will only appear when all necessary answers on the application have been completed
Once the CEU worker submits the completed questionnaire, Casebook will run internal calculations to determine whether the child is eligible or ineligible. After the determination has been made, it will display at the top of the page in the Application Card. The determination will also display in the Quick Info menu on the right hand side of the page. If the child is determined to be ineligible, CEU workers can see the reasons a child was ineligible for assistance at the top of the application.
