This is a partial screen shot of the overview/summary page for an assessment in Casebook. All Assessment overview pages have a jump menu on the left hand side and an Actions dropdown menu on the Smartbar. There is also a Quick Information menu on the left hand side. The center section of the page has progress bars and page cards.
The overview page for an assessment displays the most pertinent and up to date information for the assessment. All historical information about the assessment is storied on the History page (accessed from a link in the top right corner) and on the edit screen of the page cards.
At the top of the page, there are three progress bars.
The top bar displays the progress a FCM has made on the assessment. There are three sections on this bar:
In progress (this will read "Edit (Due to Appeal)" if the assessment has been placed in that state because the family appealed the substantiated allegation(s))

The middle bar gives workers information the assessment allegations about the response time and date the assessment was initiated on. An assessment is initiated when face-to-face contact is made and recorded with the alleged victim or telephone/face-to-face contact is made and recorded with a person (other than the alleged perpetrator) who has a relationship with the alleged victim. This bar can be three colors:
Blue: The assessment has at least one mandated reason attached to it. There are four possible mandated reasons (fatality, near-fatality, unaccompanied minor in a shelter, and a sexual predator living in the home).
Black: The assessment's initiate and due by deadlines have passed.
Green: The assessment has been initiated.
The bottom bar is the Assessment Process Timeline. Indiana DCS requires that all assessment work be completed within 40 days (except near fatalities and fatalities) from the date of the Intake. The timeline in Casebook counts down 40 days the day the assessment was created; workers will see the timeline bar turn yellow as the days pass. Once a supervisor approves a finished assessment, the countdown ceases.

The Assessment Process Timeline bar turns red when the Assessment is overdue.
Below the progress bars, you can access all of the cards for an assessment and find, enter, and edit information. Any information on each card that is required before submitting the assessment is also listed. Click on Submitting an Assessment for more information.
On an assessment, you can create an exception for some required information (click here to read more about creating exceptions):

The substantiated/unsubstantiated question must be completed before submission. A notification about entering required demographic info for the Victim and Perpetrator on their person pages is also seen in this card.
In the Report Narrative section, Reporter, Participant, Assignee, and Approvers names appear as links
