Regardless of if a youth is a member of a sibling group or IV-E eligible, if a youth wants to participate in the Collaborative Care Program his/her case will be transferred from an FCM to a 3CM.
This can be done in 3 steps:
The youth's current CHINS involvement type (not the case) will be closed for the CHINS case assigned to the FCM
A new case will be opened in MaGIK
The involvement type will be 3CM Collaborative Care/CHINS.
Creating a new case will not affect the child's eligibility. A new, collaborative care eligibility application will only be open if the older youth enters a new removal episode. To read more about the triggering event for collaborative care eligibility, click here.
Before you being, add a profile picture to the correct youth's person page. This will help you select the correct person from an autcompleter box. To ensure you're working with the right person, start at the current CHINS case page and click on the blue hyperlink that is the youth’s name.
This does not have to be an actual image of the youth.

Return to the CHINS case and close the CHINS involvement type (not the case).
Click the “Edit” link next to the “Focus Children” card on the case page.
Click the “Edit” link on the card for the youth who will enter the CC program.
Change the “What is (youth’s first name) involvement becoming” to “Closed” and click the Set as Current button. It is very important that this is done in the top box, as changing the box that currently reads “CHINS” to closed will remove the CHINS involvement history.
Adjust the date as necessary to reflect the date the youth transitioned to the 3CM
Click the save button at the bottom of the page. You do not have to click the little save button at the bottom of the child's card.
You can type magik.in.mycasebook.org/cases/new into the address bar of Firefox while logged into MaGIK. If so, skip to step 4
OR, Click the “Resources & Lists” dropdown at the top center of the page and click the “Cases” link.
Click the “Create case” link at the bottom of the page that displays all cases.
Name the Case with the name of the youth in the “Name” field
Skip the “Legacy ID” field
Skip the “Linked Assessments” field
Click the “+ Add New Youth” in the “Focus Children” card and use the auto-completer to select the name of the youth who will be the 3CM Collaborative Care/CHINS youth in the “Select the youth you want to add” field
Click the “Add New” button to save your focus child selection.
Select the appropriate county in the “County” dropdown
Use the auto-completer to select the 3CM worker the case will be assigned to (a 3CM can only assign a case to themselves, but a supervisor can assign to any user).
You will now be on the screen to create a new case
Click Save to create the new case.
When a new case is created, the current eligibility, removal episode, and placements in Casebook will follow the youth into the 3CM Collaborative Care/CHINS involvement type and case. This is because these units of work are linked to the person, not the case.

From the newly created case page, click edit on the Focus Children card.
Click the “Edit” link to the upper right of the Youth’s card
Change the “What is (youth’s first name) involvement becoming” to “3CM Collaborative Care/CHINS” and click the Set as Current button. It is very important that this is done in the top box, as changing the box that currently reads “Awaiting Approval” will remove the involvement type history.
Adjust the date to the date the case was transfered to the 3CM worker.
Click the big “Save” button at the bottom of the page.
