The Safety Assessment is a tool that allows you to quantify the safety risks in a household that can be created from a case or assessment. Multiple children can be added to the safety assessment and there are custom safety responses that allow you to record the individual efforts and agreements that the family and their supports have agreed to follow. There can only be one open and active safety assessment on an assessment or case at one time.
Completing a safety assessment for the victims on an assessment is one of the things that is required before the assessment can be submitted for approval to a supervisor. To remind you, Casebook displays a message in the Plans and Tools card on the Assessment. You also have the ability to create an exception for the safety assessment by clicking the exclamation point on the message and follow the instructions found here.
On a safety assessment questions that do NOT require an answer will display text that reads “No Answer.” This should indicate that you do not need to take further action on this question, as previous answers on the safety assessment have negated the need to answer. Questions that display "Not yet Answered" require an answer and must be completed before you can submit the safety assessment. All required fields are marked with a red asterisk.
To complete a safety assessment, you will enter information in a number of cards. The following section will go through required information by card.

If any of the questions on the Safety Threats card are answered "yes" then all questions on this card are required.
If all questions on the Safety Threats card are answered "no" then none of the questions on this card are required.
If the 7th question on the Safety Response card is answered "yes" then the "Removed from Information" is required on the Safety Decision card.
If any of the first six questions on the Safety Response card are answered "yes" then the "Conditionally Safe Narrative" is required.
If any of the questions on the Safety Threat card are answered "Yes" and questions 1-6 on the Safety Response card are answered no, then the "Removed from Information" is required is required on the Safety Decision card.
This card lists an automatically selected safety level for the child according to the other answers from the quiz. If you have not answered all of the questions from the Safety Threats card, a message appears that reads, "MaGIK cannot calculate the risk level until you have given an answer for all the possible safety threats." Once all required questions have been answered, a safety decision will display in this card.
This card is found on the bottom of the Safety Assessment and needs be checked to indicate the assessment is complete (or marked as final) before submitting. The checkbox will be disabled and not clickable until all required information on the Safety Assessment is properly filled out. The Safety Assessment will be able to be saved, but not marked as Final until all required questions have been answered.
The screen shot below displays a partial view of the Safety Assessment. Like other SDMs, the top right corner has a link that will take you back to the overview page of the case or assessment.
Upon completion of the Safety Assessment, when you click Save, a pop-up box will appear asking you to confirm that everything on the assessment has been completed. If so, click Submit. This popup box will not appear until all required questions have been answered, however, you will still be able to click the save button before all required questions have been answered to save your work.
If you try to submit the assessment before all questions have been answered, the standard 3 error messages will appear.
A red outlined box will appear at the top of the page that lists which required questions are unanswered.

Workers have 7 days to edit the assessment once it has been submitted.
Like other SDMs, you can delete an existing Safety Assessment from the plan's edit screen.
Click on the linked name of an existing Safety Assessment from the Plans and Tools card on a Case or Assessment.
From the Actions Menu, select Delete. You will be able to delete the Safety Assessment anytime before has been submitted and has been marked as final.
A popup will appear asking if you are sure you want to delete the Safety Assessment. Select yes to delete the assessment and return to the main overview page of the case or assessment.
