ฤทธิ์เป็น parasym: กระตุ้นกล้ามเนื้อเรียบ หูรูดคลายตัว

การออกฤทธิ์

Bethanechol เป็นยาในกลุ่ม chlorinergic drug (parasympathomimetics)

จะออกฤทธิ์โดยการกระตุ้นผ่าน muscarinic receptor มีผลต่อทางเดินปัสสาวะและทางเดินอาหาร

ผลต่อทางเดินปัสสาวะ ทำให้

-กล้ามเนื้อ detrusor ของกระเพาะปัสสาวะบีบตัวมากขึ้น peristalsis ของท่อ ปัสสาวะเพิ่มขึ้น

-ลดความจุของกระเพาะปัสสาวะ

-ส่วน trigone และ sphincter ของกระเพาะปัสสาวะจะคลายตัว

ผลต่อทางเดินอาหาร

-สามารถเพิ่มแรงตึงตัว แรงบีบตัว การเคลื่อนไหวแบบ peristalsis ของกระเพาะอาหารและลำไส้

-กล้ามเนื้อหูรูด (sphincters) คลาย

-เพิ่มการหลั่ง secretion ต่างๆ ในทางเดินอาหาร

ข้อบ่งใช้ในการรักษา

-ภาวะที่เกิดการคั่งของปัสสาวะหลังการผ่าตัด และ การคลอด

( acute post-op & postpartum non-obstructive urinary retention. )

-โรคเกี่ยวกับกระเพาะปัสสาวะไม่ทำงาน : Neurogenic atony ของกระเพาะปัสสาวะ

หรือ ปัสสาวะบ่อยๆ เนื่องจากกระเพาะปัสสาวะไม่ค่อยบีบตัว ( Frenquency)

-ยังใช้กระตุ้นการทำงานของระบบทางเดินอาหารหลังการผ่าตัด หรือกรณี gastric atony, gastroparesis

ขนาดยา

Adult 10-50 mg/day in 3-4 divided doses

Ucholine Sig. 5 mg 1-3*3 pc.

การรับประทาน

ควรกินในขณะท้องว่าง

Should be taken on an empty stomach

กินตอนท้องว่าง ก่อนอาหาร 1 ชั่วโมง หรือ หลังอาหารแล้ว 2 ชั่วโมง

(Take on an empty stomach 1 hr before or 2 hr after meals.)

ข้อห้าม

ทางเดินปัสสาวะหรือทางเดินอาหารอุดตัน

ADVERSE DRUG REACTIONS

ไข้ ครั่นเนื้อครั่นตัว

ปวดบีบลำไส้ คลื่นไส้ ไม่สบายท้อง ท้องอืด

INTERACTIONS

Ganglion blockers.

INDICATIONS

Treatment of acute post-op & postpartum non-obstructive urinary retention. Neurogenic atony of the urinary bladder w/ retention.

DOSAGE

Adult 10-50 mg/day in 3-4 divided doses.

ADMINISTRATION: Should be taken on an empty stomach (Take on an empty stomach 1 hr before or 2 hr after meals.).

US FDA PREGNANCY CATEGORY C

CONTRAINDICATIONS

Avoid use when the strength or integrity of the GI or bladder wall is compromised or in the presence of mechanical obstruction.

ADVERSE DRUG REACTIONS

Malaise; digestive abdominal cramps or discomfort, colicky pain, nausea & belching.

INTERACTIONS

Ganglion blockers.
