Indication

Last line drug ดื้อยาหลายชนิด

Empiric therapy:Nosocomial infection,suspected from MDR gm neg bacilli or Mixed infection

Specific therapy : gm neg resist to 3^rd gen Ceph,AMG and sens to Tienam,Meropenem

Indication : ผู้ป่วยมีความเสี่ยงต่ออาการชักจากการใช้ยา Tienam®หรือ / และ เชื้อที่ดื้อต่อยา Tienam

Empiric therapy

1. ภาวะ febrile neutropeniae

2. การติดเชื้อ Nosocomial ที่ไม่ตอบสนองต่อการรักษาด้วยยากลุ่มอื่น

suspected from MDR gm-neg bacilli or Mixed infection

3. สำหรับการติดเชื้อที่อาจเกิดจาก P. aeruginosa ระหว่างรอผลการตรวจหาเชื้อ

Specific therapy

1. มีการติดเชื้อ Polymicrobial ที่มี ESBL strain ร่วมด้วย

2. มีการติดเชื้อmulti drugs resistance หรือ pan-resistance

คือ gm neg resist to 3^rd gen Ceph,AMG and sens to Tienam,Meropenem

**ESBL รักษาได้โดยใช้ยากลุ่ม นี้เท่านั้น

**จริงๆ ESBL drog of choice คือ Ertapenam แต่ราคาแพง

อาจให้ mero, imipenam แทนได้

ขนาดยา

Adult

1-2 gm แบ่ง 3-4 ครั้ง

Ex :

500 mg iv drip 30 min q 8 hr (>500 drip in 60 min)

1gm iv drip 60 min q 8 hr (>500 drip in 60 min)

Max 4 gm/day, 50mg/kg/day

Pediatric

> 3 เดือน 15-25 mg/kg/dose q 6 hr

< 1wk 25mg/kg q 12 hr

1-4 wk 25mg/kf q 8 hr

4wk-3m 25 mg/kg q 6 hr

Renal dose impairment

>70 dose ปกติ q 6 hr

30-70 dose ปกติ q 8 hr

20-30 dose ปกติ q 12 hr

<20 dose 50% q 12 hr

Precaution

1.cross allergy กับ beta-lactam, penicillin, cephalosporin

2.pseudomonas colitis

3.CNS : ชัก สับสน เกร็ง
