1.รักษา unstable VT

ก็ใช้ loading 150 mg push ช้า ๆ ใน 10 นาที แล้วตามด้วย maintenance 1000 - 1200 mg IV drip ใน 24 ชั่วโมง

An initial intravenous loading dose of 150 mg is given over 10 minutes followed by a continuous infusion of 1 mg/min for six hours and 0.5 mg/min thereafter (all 1050 mg in 24 hr)

2. รักษา VF ก็ยังใข้ 5 mg/kg เหมือนเดิม max ที่ 300 mg IV push ช้า ๆ แล้วต่อด้วย 1200 mg IV drip ใน 24 ชั่วโมง

Intravenous amiodarone is also effective in patients with persistent ventricular fibrillation who have not responded to three shocks with the defibrillator. The usual dose is 5 mg/kg to a maximum of 300 mg diluted to 30 mL with dextrose in water and given rapidly into a peripheral vein

Loading dose ?

อะมิโอดาโรน มีค่าครึ่งชีวิตที่ยาวทั้งยังมีความแปรผันมากในแต่ละบุคคลที่ทำให้ค่าครึ่งชีวิตแตก ต่างกัน (20-100 วัน) ในระหว่างวันแรกของการรักษา ยาจะสะสมในเกือบทุกเนื้อเยื่อโดยเฉพาะในเนื้อเยื่อไขมัน การขจัดยาจะเกิดขึ้นภายหลัง 2-3 วัน และระดับยาในเลือดถึงระดับคงที่ในระหว่าง 1 เดือนถึงหลายเดือน ขึ้นกับคนไข้แต่ละราย เนื่องจากคุณลักษณะที่กล่าวมาข้างต้น จึงควรให้ยาโดยการโถมยา (loading dose) เพื่อให้ได้ผลที่รวดเร็ว

Diluted ?

Amiodarone should be mixed in a 5 percent dextrose solution and the amiodarone concentration kept below 2 mg/mL if given through a peripheral vein to minimize the development of local phlebitis. Higher drug concentrations must be delivered through a central vein. Amiodarone is physically incompatible with a number of drugs, including heparin, which should not be given in the same solution.
