CONTENTS Equine tetanus antitoxin
INDICATIONS
Prophylaxis & treatment of tetanus.
DOSAGE
Prophylaxis 1,500 iu SC/IM. Double or triple dose in severe wounds.(1500-5000 IU im)
Indication
ผู้ป่วยได้รับการฉีด tetanus toxoid ไม่ครบชุด หรือไม่เคยฉีดมาเลยหรือไม่ทราบว่าเคยฉีดมาก่อนหรือไม่ ให้ฉีด tetanus toxoid 1 ชุด และ บาดแผลฉกรรจ์ให้ฉีด antitoxin ให้
SPECIAL PRECAUTIONS
Perform hypersensitivity testing.
Skin test
การฉีด tetanus antitoxin ควรถามประวัติการแพ้ต่าง ๆ หรือประวัติเป็นลมพิษและต้องทำการทดสอบผิวหนัง (intracutaneous sensitivity test) ก่อนทุกครั้ง
การทดสอบการแพ้ซีรั่มโดยวิธีฉีดเข้าในหนัง (intracutaneous) อาจทำได้โดยเจือจาง
Tetanus antitoxin 0.1 cc + ดูด normal saline เข้าไปจนครบ 10 มล. จะได้น้ำยา 1 : 100
แล้วฉีดเข้าในหนังขนาด 0.1 มล. จากนั้นเฝ้าดูอาการเป็นเวลา 10-30 นาที ถ้ามีอาการแพ้ หนังบริเวณที่ฉีดจะมีรอยนูนแดง โดยมีส่วนที่นูน (wheal) เกิน 1 ซม.
ในการฉีดวัคซีนทุกชนิดไม่ว่าจะเป็นการฉีดเข้าในหนังเพื่อทดสอบอาการแพ้หรือฉีดเข้าใต้หนังเพื่อป้องกันหรือรักษาโรคก็ตาม ต้องเตรียม adrenaline ไว้ให้พร้อมที่จะใช้ได้ทันทีเมื่อผู้รับการฉีดเกิดแพ้
การฉีด tetanus antitoxin ต้องฉีดคนละข้างกับ tetanus toxoid และไม่ผสมปนในกระบอกฉีดเดียวกัน เพราะจะทำให้เสื่อมฤทธิ์
ADVERSE DRUG REACTIONS
Anaphylactoid reactions w/ rashes eg, urticarial rash, itching, joint pains, slight edema.

Tetanus Immune Globulin

Tetuman หรือ Tetagam คือ Tetanus Immunoglobulin (TIg) ผลิตจากคน
ทั้ง 2 ยี่ห้อ ขนาด 250 IU เท่ากัน มีข้อดีตรงที่ไม่ต้องทำ Skin test ก่อนใช้ และไม่เกิดปฏิกิริยา Hypersensitivities เหมือน TAT รุ่นเก่าขององค์การฯที่ทำมาจากม้าน่ะค่ะ
การใช้ ถ้าใช้ในกรณี prophylaxis ก็ใช้ได้ในกรณีที่แผลมีการเสี่ยงต่อการติดเชื้อ โดยสามารถให้ร่วมกับ Tetanus vaccine ได้
Dosage and Administration
Adults Prophylactic dose
IM 250 units. Give 500 units if wounds are severe or treatment is delayed. Dosage may be increased to 1,000 to 2,000 units.
For therapy of tetanus, give 500 to 3,000 or 6,000 units. Give deep IM, preferably in upper outer quadrant of gluteal muscle.
Children
IM Dose is calculated on basis of body weight (4 units/kg); however, it may be advisable to administer 250 units regardless of the size of the child. The same amount of toxin is produced by the bacteria in adults and children.
General Advice
Refer to appropriate immunization schedule or CDC wound management guideline for correct dosing.
Use different syringe and injection site for the tetanus toxoid.
Pharmacology
Directly neutralizes toxin excreted by Clostridium tetani , cause of tetanus.
Pharmacokinetics
Absorption
Tetanus immune globulin T max is approximately 2 days after IM injection.
Elimination
Tetanus immune globulin T ½ is 23 days.
Indications and Usage
Passive, transient protection against tetanus in any person that may be contaminated with tetanus spores when: (1) patient's personal history of immunization with tetanus toxoid is unknown or uncertain, (2) person received less than 2 prior doses of tetanus toxoid, or (3) person received 2 prior doses of tetanus toxoid, but delay of more than 24 h occurred between time of injury and initiation of tetanus prophylaxis.
Unlabeled Uses
Treatment of clinical tetanus.
Contraindications
Hypersensitivity to human antibody product, thimerosal, or other components; circulating anti-IgA antibodies.
Storage/Stability
Store in the refrigerator.
Drug Interactions
There is no significant interaction between TIG and tetanus toxoid if given at different injection sites. To avoid inactivating vaccines containing live viruses or bacteria, give live vaccines 2 to 4 wk before or 12 wk after TIG.
Laboratory Test Interactions
None well documented.
Adverse Reactions
Local and systemic reactions following TIG are infrequent and usually mild. Expect some pain, tenderness, and muscle stiffness at injection site, persisting for several hours. Hives, angioedema, nephrotic syndrome, and local inflammation occur occasionally. Anaphylactic reactions are very infrequent.

Category C .
Lactation
Undetermined. Use TIG as soon as possible after tetanus-prone injuries. Do not inject IV.
