Meropenam

Meropenam (Meronam® ขนาด 500mg, 1 gm)

Indication : ผู้ป่วยมีความเสี่ยงต่ออาการชักจากการใช้ยา Tienam®หรือ / และ เชื้อที่ดื้อต่อยา Tienam

กา่รเลือก : ควรใช้ imipenam ก่อนหากดื้อแล้วค่อยให้ meropenam

การออกฤทธิ์

เป็น broad spectrum carbapenam antibiotic : gm+,gm-,anaerobe ใช้เป็น monotherapy ได้ดี

ยับยั้งการสังเคราะห์ cell wall ใน gm + และ gm – bacteria

Empiric therapy

1. ภาวะ febrile neutropeniae

2. การติดเชื้อ Nosocomial ที่ไม่ตอบสนองต่อการรักษาด้วยยากลุ่มอื่น

suspected from MDR gm-neg bacilli or Mixed infection

3. สำหรับการติดเชื้อที่อาจเกิดจาก P. aeruginosa ระหว่างรอผลการตรวจหาเชื้อ

Specific therapy

1. มีการติดเชื้อ Polymicrobial ที่มี ESBL strain ร่วมด้วย

2. มีการติดเชื้อmulti drugs resistance หรือ pan-resistance

คือ gm neg resist to 3^rd gen Ceph,AMG and sens to Tienam,Meropenem

**ESBL รักษาได้โดยใช้ยากลุ่ม นี้เท่านั้น

**จริงๆ ESBL drog of choice คือ Ertapenam แต่ราคาแพง

อาจให้ mero, imipenam แทนได้

ประสิทธิภาพ

intraabdominal infection

Meropenam 1m q 8 monotherapy > Cefotaxime 2gq8+metronidazole500mgq8

หญิงตั้งครรภ์ : Cat. B

ข้อระวัง

-การแพ้ยาร่วมกันได้ meropenam, carbapenams, penicillins, cephalosporin

-ห้ามใช้ยา meropenam ในการรักษา Pseudomonas aerugenosa ใน LRI

-ปรับยาในผู้ป่วยโรคไต

Site effect

GI: Nausea, vomiting, diarrhea, increases in liver enzymes and bilirubin

Immune : positive Coomb's test, eosinophilia,thrombocytopenia, neutropenia, hypersensitivity (rash, pruritus,urticaria),

Neuro: convulsions, confusion, hallucinations

injection site (erythema, pain,thrombophlebitis)

การบริหารยา&เตรียมยา :

1. เติม SWFI 20 ml ใน Meropenem inj . 1 gm (conc.50mg/ml)

2. IV push (> 5 นาที) , IV infusion (15-30 นาที)

สารละลายที่เข้ากันได้ :

1. 0.9%NSS สารละลายที่เจือจางแล้วจะมีความคงตัว 48 ชม. ที่อุณหภูมิห้อง (4°C)

2. D5W, D5S สารละลายที่เจือจางแล้วจะมีความคงตัว 14 ชม. ที่อุณหภูมิห้อง(4°C)
