Internal referrals for all programs can now be entered on the Internal Referral form in axiUm.The Internal Referral form in axiUm now contains a new Perio Grad tab. For all referrals to Perio Grad - please use this form.Â
The previous Perio Graduate Referral Form has now been inactivated and will be accessible as read only. Â
Below is a checklist of items for OHC proper operatory set-up
Place the Mayo cart under the counter
Computer is placed on left side of operatory
Assistant chair on left side
Operator stool on right side
Raise the dental chair high enough to have all cords off of the floor
Position dental light at its highest point facing out
Rheostat stays on the floof not on the chair base
Hand mirror is caviwiped, rinsed, dried and put on the high hook
Light shield is caviwiped, rinsed, dried and put on the low hook
Amalgamator is to be on the counter
Blood pressure cuff is to be left on the counter
Turn off the chair master switch at the end of each clinic
Operator and Assistant chairs are assigned to specific operatories throughout the clinic. If a chair is borrowed from an operatory ensure it is returned to where it was borrowed from.Â
CDE is looking for new patients for the IV Sedation program. This program is ideal for individuals who:
Are healthy ADULT patients
Do not have dental benefits
Do not have existing health or medical concerns
Are not pregnant
May be somewhat anxious of dental treatment – although you do not necessarily need to be an anxious patient. This program is open to anyone who is eligible.
Have a BMI that is below 35 at the time of screening and treatment in order to proceed. *An accurate measure of height and weight is required as part of the screening process for this program.
Must be willing to receive IV Sedation for every appointment.
