REMINDER: Annual Influenza Vaccination

Once again, the School of Dentistry has partnered with Rexall Pharmacy in KEC to provide flu vaccinations, as well as the COVID-19 booster vaccination upon request, to SoD students, faculty and staff.

Rexall Pharmacy staff will be available during the following dates/times in the Oral Health Clinic (Oral Surgery - operatory S135).  In order to expedite matters, please complete this Rexall Vaccination Consent Form in advance.

• Tuesday, October 17th 

• 7:30am-9:00am & 12:00pm-1:00pm

• Wednesday, October 18th  

• 7:30am-9:00am & 12:00pm-1:00pm

REMINDER: Laminated Medical/Dental History Forms

Each operatory should have laminated Medical/Dental History forms for patients to reference.  The laminated forms are stored in the operatory cupboard (hook on the inner surface of the cupboard door).  Please remember to return the forms to this location after use.

REMINDER:  OHC Scent Free Zone

Please remember that the OHC is a scent-free zone - no heavily scented products, perfumes or colognes should be worn by students/faculty/staff.
