Students must ensure that the Consent to Treatment & Financial Agreement is signed by the Patient once a treatment plan is reviewed with and consent to by the Patient. Only once the Consent to Treatment & Financial Agreement is signed will the treatment appear as "planned" in the Treatment History of the EHR in axiUm. Â
Students must "tie" planned treatment to appointment requests as these are often booked by Patient Services Clerks at check out. Â
"Tying" planned treatment to appointments facilitates automatic equipment requests - no need for manual equipment requests in these cases. Â
Students who have patients without planned treatment will need to submit a manual equipment request.
Please review all equipment requests at least 48 hours prior to the appointment - if additional equipment is required, students must add a manual request for the additional equipment 48 hours in advance of the scheduled appointment.
