Swipe Card Readers - Tip Swipe card not working for approvals? Please check if CAPS LOCK is turned on. If so, please turn it off and try again.Â
Completing Treatment Codes Please ensure all treatment codes are completed (and approved if applicable), prior to dismissing your patient. Failing to do so results in delays at check out.
ATTENTION: CDCP Patient Requiring Additional Units of Scale/Root Planning (in excess of 4 units/year)
CDCP has been approving additional units of scaling for patients with three or more pockets of 3mm or more. Currently they will approve either 2 or 4 additional units for each year for the next two years.
Pre-authorization is required in advance of treatment, and additional information is required to be included in the submission - including x-rays (within a year), full mouth probing and a medical rationale.
Please complete the Pre-Determination Form in the EHR to initiate the pre-authorization.Â
Some sections of the PRE-D form in axiUm forms will auto-populate with information from the perio chart and the perio form (CAF).
When outlining "treatment plan for the next 24 months", please select both IPT and MSPT if applicable. Enter the number of SC/RP units the patient will require for each round of therapy, the number of anticipated rounds of therapy, as well as the re-evaluation/recall intervals.
Eg. P3 patient with active disease:
IPT: 6 units
MSPT: 4 units
interval: 3 months
When this PRE-D form is completed and approved (no approval required for PG), an automated message will be sent via axiUm to the finance team. They will then submit the pre-authorization along with all required documentation (radiographs, perio charting and the letter generated through the PRE-D form). Â
Please ensure this form is completed for ALL CDCP patients.Â
Pre-authorizations for additional units of scale will take a minimum of 5 weeks for CDCP to process.
