REMINDER: Clinic VisitorsÂ
If you plan to have a company representative visit the clinic, please review the Inviting Visitors and Volunteers page in the Dentranet. You will need to complete the School of Dentistry Visitor Request Form.
REMINDER: Treatment-specific AARC Forms (Acknowledgement of Additional Risks & Considerations) are required for Patients (or legal guardians/signing authorities) to sign, along with the Consent to Treatment & Financial Agreement once treatment is planned. Â
Once the Patient consents to the treatment plan, please look for red "Do Not Enter" signs (in the axiUm Treatment History) adjacent to treatment codes requiring treatment-specific AARC forms signed.

PROCESS/WORKFLOW:
Treatment Provider plans treatment
Supervising faculty approves treatment plan
Treatment Provider reviews draft of Consent to Treatment & Financial Agreement with Patient, including fees and basic risks and considerations
Patient consents to treatment by signing Consent to Treatment & Financial Agreement
Treatment Provider identifies need for treatment-specific AARC Form (red "Do Not Enter" sign in axiUm treatment history)
Treatment Provider reviews each treatment-specific AARC form (there may be several) with Patient ie. Additional Risks & Considerations section
Patient acknowledges treatment-specific additional risks and considerations, and signs AARC Form
When Treatment Provider accompanies Patient to Patient Services for "check out", PS Clerk will ask Treatment Provider if any AARC forms need to be printed or email out to Patient (Patient will provide PS Clerk with print/email preference)
Hopefully, this will provide the Patient with some time to digest the information prior to coming in for higher risk treatment.

The following are additional authorization forms that are specific to orthodontic treatment:
AUTHORIZATION FOR EARLY TERMINATION OF ORTHODONTIC TREATMENT (ORAL HYGIENE)
AUTHORIZATION FOR EARLY REMOVAL OF BRACES / TERMINATION OF ALIGNER THERAPY (PT REQUEST)
AUTHORIZATION FOR REMOVAL OF BRACES / TERMINATION OF ALIGNER THERAPY (STANDARD)
The following are additional authorization forms that will be available for use, as required:
AUTHORIZATION FOR ASSIGNMENT OF CONSENT
AUTHORIZATION TO RECORD VISIT WITH TREATMENT PROVIDER
CONSENT TO REPRODUCE PHYSICAL LIKENESS, VOICE AND/OR NAME FOR MARKETING
Always confirm with the Patient chairside, on the day of procedure, that the Patient has reviewed and understood risks and considerations.Â
Ask the Patient if they have any questions.
Ask the Patient if they are still willing to undergo the procedure. Â
If Patient agrees to proceed with treatment, the Treatment Provider documents the following in the EHR Tx Notes:Â "Reviewed risks and considerations with Patient and gave Patient the opportunity to ask questions. Patient chose to proceed with treatment."

ATTENTION DDS Students: Lab Shelf and Loaned Item Cleanup
DDS Students are required to transfer their items from their KEC Student Lab shelves into their designated assigned lab lockers.
Transfer must be completed by February 5, 2025
Any items left on the shelves after this deadline will be discarded
