The OHC has transitioned to end tuft brushes instead of Sulca Brushes and these are now available in both Dispensaries.Â
Pre-authorizations for CDCP Schedule B treatment can now be submitted. Additional information is required for all patients requiring scaling and root planing in excess of 4 units per year. Pre-authorizations for periodontal treatment estimated to occur in a 2 year time frame can be submitted.
Some sections of the PRE-D form in axiUm forms will auto-populate with information from the perio chart and the perio form (CAF).
Medical rationale is also required in the PRE-D form.
When outlining "treatment plan for the next 24 months", please select both IPT and MSPT if applicable. Enter the number of SC/RP units the patient will require for each round of therapy, the number of anticipated rounds of therapy, as well as the re-evaluation/recall intervals.
Eg. P3 patient with active disease:
IPT: 6 units
MSPT: 4 units
interval: 3 months
When this PRE-D form is completed and approved (no approval required for PG), an automated message will be sent via axiUm to the finance team. They will then submit the pre-authorization along with all required documentation (radiographs, perio charting and the letter generated through the PRE-D form). Â
Please ensure this form is completed for ALL CDCP patients.Â

Keri Pilon is arranging a donation to the food bank, more than ever food donations are significantly needed, and I was hoping for your support to help fill my Mini Cooper with food donations. There is a donation box located just outside the Materials Management/Stores door in the back hallway by the staff/student elevators for anyone who would like to drop off non-perishable items, until Dec.12.
I have attached a list of the food banks most needed items, if you aren't sure what to bring.

Santa’s Anonymous is often short of items for infants to 2 year olds and 9 to 12 year olds.
Toy donations should be new unwrapped toys. Toys with lots of play value make the very best gifts! If a toy can be played alone or with another child, can be played in many different ways and doesn’t need batteries or electronics - it has lots of play value!Â
