Starting January 3, 2023, treatment-specific AARC Forms (Acknowledgement of Additional Risks & Considerations) will be required for Patients (or legal guardians/signing authorities) to sign, along with the Consent to Treatment & Financial Agreement once treatment is planned. Please note that current treatment-specific consent forms used in the Periodontology Graduate Program, as well as how they are used, will not be changing for now.Â
Once the Patient consents to the treatment plan, please look for red "Do Not Enter" signs (in the axiUm Treatment History) adjacent to treatment codes requiring treatment-specific AARC forms signed.
PROCESS/WORKFLOW:
Treatment Provider plans treatment
Supervising faculty approves treatment plan
Treatment Provider reviews draft of Consent to Treatment & Financial Agreement with Patient, including fees and basic risks and considerations
Patient consents to treatment by signing Consent to Treatment & Financial Agreement
Treatment Provider identifies need for treatment-specific AARC Form (red "Do Not Enter" sign in axiUm treatment history)
Treatment Provider reviews each treatment-specific AARC form (there may be several) with Patient ie. Additional Risks & Considerations section
Patient acknowledges treatment-specific additional risks and considerations, and signs AARC Form
When Treatment Provider accompanies Patient to Patient Services for "check out", PS Clerk will ask Treatment Provider if any AARC forms need to be printed or email out to Patient (Patient will provide PS Clerk with print/email preference)
Hopefully, this will provide the Patient with some time to digest the information prior to coming in for higher risk treatment.
Please familiarize yourself with the following AARC forms before they are implemented on January 3, 2023:

Always confirm with the Patient chairside, on the day of procedure, that the Patient has reviewed and understood risks and considerations.Â
Ask the Patient if they have any questions.
Ask the Patient if they are still willing to undergo the procedure. Â
If Patient agrees to proceed with treatment, the Treatment Provider documents the following in the EHR Tx Notes:Â "Reviewed risks and considerations with Patient and gave Patient the opportunity to ask questions. Patient chose to proceed with treatment."
New fees in the DDS/DH Fee Guide go into effect January 3, 2023.Â
Most treatment, with some exceptions*, has undergone an approximate 6% increase - similar to the increase found in the 2023 ADA Fee Guide for general dentists.
*exceptions include crowns, bridge retainers, pontics, dental implants (restorative & surgery), definitive complete dentures. Please also note that three lab options, with 3 different costs, will be available for the treatment provider to choose from when planning ceramic restorations ie. full contour zirconia, pressed e.max, or layered zirconia/e.max.
As of January 3, 2023, only ASTM level 3 masks will be available in the Level 1 and Level 8 KEC back corridors by the staff elevators. There will be no need to change out of this mask before starting clinical procedures.
As long as scrubs meet IPC requirements and are professional and appropriate, DDS and DH students (in all* years of the programs) may wear "fun" scrubs, on Fridays only, starting Winter term 2023 in both the OHC and the Sim Lab.Â
*Please note that DH3 students can wear their "fun" scrubs on Tuesdays during Winter Term, since they are not in the OHC on Fridays :-)
Scrubs must be worn in the OHC and Sim Lab - we don’t want to see t-shirts, hoodies, jeans, khakis etc. creeping into the Sim Lab or OHC.
