It is imperative that patients sign their own consent forms prior to providing them with any treatment. Â
Faculty/staff/students must NOT sign any consent forms on behalf of the patient under any circumstances.
An additional fee increase to most basic restorative and oral surgery procedures will become effective Monday, January 22, 2024 in order to offset the reduction to fees for tooth supported crowns, pontics and bridge retainers. Please see the DDS/DH Fee Guide for details. Â
Please note that patients who had previously consented to tooth-supported crown and bridge procedures will be contacted and informed of the fee reduction by Comprehensive Care Treatment Coordinators, only if the disease control phase of treatment has been completed.
Fees associated with oral surgery and basic restorative procedures that were consented to by the patient prior to the fee increase will be honoured.
Students are reminded to use the predefined macro codes found in axiUm's Quick Lists when treatment planning periodontal treatment related to the patient's periodontal classification. The number of scaling/root planing units should only be modified when the patient has less than a full complement of teeth. Please refer to the Periodontal Debridement Fee Structure for details.
