Dear Staff, Faculty and Students,
We are days away from the end of winter term and after a short break, many of us will be back for intersession. Now is not the time to let up. We need to remain focused and vigilant with our infection prevention & control so that we can continue to learn and provide patient care in our various locations.
While contact tracing recent case(s), we’ve discovered that many have been forgetting to complete their Fit for Work (FFW) screening form for each in-person clinical or simulation session.Â
If you are on campus or attending any of our external sites for morning, afternoon and evening you must complete the following:Â
1) An initial FFW form in the morning before you leave your house indicating AM and where you will beÂ
2) A second FFW form at lunch indicating PM and where you will beÂ
3) A third FFW form indicating AFTER 5:00pm and where you will be before beginning your evening session.Â
If you are volunteering with SHINE Dentistry on the weekend, you must also complete the FFW screening form prior to attending the Boyle McCauley Dental Clinic. Completion of these forms is required for regulatory compliance and also to ensure accuracy of contact tracing and notification should there be a positive case reported.

Please remember:
Before leaving home for campus/external site or beginning another clinic or lab session complete a FFW Screening form. You must complete a new FFW screening form for every clinical or simulation in-person session as follows:
Morning clinics/labs/external rotations/volunteering 8-12 pm
Afternoon clinics/labs/external rotations/volunteering 1-4 pm
Evening clinics/labs/external rotations/volunteering After 5:00 pm
Hand hygiene must be done EVERY TIME you don, doff or touch your mask and before donning and after doffing gloves.
When in KEC or in the Simulation Lab in Kipnes ensure you are wearing a medical mask and safety glasses or a full face shield the entire time you’re there.
When in Kipnes (outside of the SimLab) UAH or Katz ensure that you are wearing a medical mask (safety glasses are a good idea as well) at all times
