Tutorial done in PSPX2.

open your template and delete the copyright layer.
add a new raster layer below the floor layer.
select all.
open paper6 from the kit.
copy and paste (C/P) into selection on the new layer.
select none.
duplicate it and pull the duplicate below the Backdrop layer.
hide it.
now use your pick tool to pull the down horizontally.
pull it down like the example below:
now still using your pick tool, hover it above the upper left node on the paper.
you'll see a rectangle shape.
hold down the CTRL key and hover over the node again and you'll see the shape change.
click the node while holding down the CTRL key and pull it in towards the center of the image.
now select the floor layer and float/defloat/invert selection.
hide the floor layer and hit delete on the paper layer.
select none.
now unhide the duplicate paper layer below the backdrop layer.
select the backdrop layer and float/defloat/invert selection.
hide delete on the duplicate paper layer and select none.
hide the backdrop layer.
add a new raster layer above the duplicate paper layer.
select your paintbrush on your toolbar.
select #000000 as your foreground color and give your paintbrush these settings:
on the new raster layer, paint out the following shape.
now using gaussian blur, blur by 50.
unhide your backdrop layer and float/defloat/invert selection.
hide delete on the paintbrush layer.
select none and hide backdrop layer.
now add any other elements and tubes that you would like.
as for the mask layer, add a new raster layer above the background layer.
flood fill with #000000.
apply the mask with default settings.
when you're all done, copy special > copy merged.
paste as a new image and use your magic wand tool to select outside the tag.
invert selection and crop to selection.
that will get rid of any extra space that you don't need.
