Tutorial done in PSPX2.

open a new image with tranparent background, 750px by 650px.
use your circle preset shape to make a large oval.
select symmetric shape in your preset shapes flyout menu.
select black to fill your background and foreground.
go to objects > align > center in canvas.
rasterize the oval.
add a new raster layer above the oval and select all.
select paper1 from the kit.
copy and paste it into selection on new layer.
select the oval layer and float/defloat/invert selection.
hit delete on the paper1 layer.
select none and hide oval layer.
open frame8 from the kit, resize by 70%.
copy and paste it as a new layer above the paper1 oval.
open frame3 from the kit, resize by 70%.
copy and paste it as a new layer below frame8.
add a new raster layer below frame8.
flood fill with this color: #e1bd85.
under effects > texture effects > blinds, use these settings:
width - 5
opacity - 30
color - #ffffff
horizontal - checked
light from left/top - unchecked
open element 25 from the kit and position it on the bottom of frame8 so the tube
looks like she is sitting.
use your magic wand to select the area inside frame8.
expand by 2 and invert.
hit delete on the brown fill layer and element 25.
now add your tube to the middle of the canvas.
add a new layer below paper1 oval and select all.
open paper5 from the kit and copy and paste into selection on new layer.
select none.
apply the mask at default settings.
right click on the mask layer in the layer palette to merge mask group.
now that all the basics are done, add any elements, text and tubes.
don't forget to add artist credit and license info.
when you're all done, copy special > copy merged.
paste as a new image and use your magic wand tool to select outside the tag.
invert selection and crop to selection.
that will get rid of any extra space that you don't need.
