Definition

Neuraxial anesthesia는 교감신경계와 체성 신경계를 억제시키므로, 보상성 반사와 부교감신경의 활성화가 나타날 수 있다.

Cardiovasclular

• Stroke volume
  • Venous and arterial vasodilation으로 인해 preload가 감소하여 SV 감소
  • Neuraxial blockade후 vasodilatory changes는 CO변화에 영향을 주며 환자의 baseline

sympathetic tone이 높을수록 혈역학적 변화가 크다.

• 건강한 환자에서 전신혈관저항은 15~18%감소하며, 심질환이 있는 노인의 경우 부위마취 후 CO는10%가량 감소하는 반면, 전신혈관 저항은 25%까지도 감소한다,.
• HR 감소
  • Extensive peripheral sympathectomy (T5-L2)가 있을 때, lower limb & abdominal/pelvic viscera의 venous pooling에 의해 발생.
  • Venous return & atrial filling의 감소로 우심방의 chronotropic stretch receptor의 signal output이 감소하나, hypotension에 의한 baroreceptor의 보상작용이 있어 HR의 감소는 최소한으로 일어남.
  • Neuraxial anesthesia가 T1까지 일어나는 경우 T1~T4에서 발생하는 cardioaccelerator fiber의 차단에 의해 venous return이 심하게 감소될 수 있고, 그 결과 심한 서맥이나 asystole까지 발생 가능.

Treatment

• Ephedrine (a mixed adrenergic agonist)
• Block 후 aBP감소는 IV crystalloid로 최소화할 수 있으나 valid concept는 아님.
• Block전 250~2000mL hydration은 일시적으로 preload, CO를 증가시키나 지속적으로 aBP를 증가시키는 방법.은 아님.
• Continuous spinal catheter를 통해 적은 양의 국소마취제를 반복적 bolus주입하여 저혈압 예방.
• Small dose unilateral 또는 selective small dose spinal anesthesia로 BP감소를 예방할 수 있음.
